胃癌发生发展中DNA倍体分析与Fas表达的临床价值
Correlativity of DNA Ploidy Analysis and Fas Expression in Human Gastric Carcinogenesis
【摘要】 目的研究DNA倍体分析与Fas表达在胃癌发生、发展过程中的动态变化及二者关系。从定量病理学、临床免疫学角度探讨细胞增殖和细胞凋亡在胃癌发生发展中的临床意义。方法采用FCM对四种病变组织经胃镜采取的活检胃粘膜组织(经病理诊断)进行DNA倍体和FAS表达检测。结果(1)中-重度不典型增生病变和胃癌DNA异倍体率显著高于慢性萎缩性胃炎和轻度不典型增生(P<0.01);(2)胃癌DI值显著高于慢性炎症和不典型增生(P<0.01);四种病变DI值显著高于正常胃黏膜(P<0.01);轻度不典型增生显著高于慢性炎症(P<0.05)。(3)在无异倍体标本中,胃癌、中、重度不典型增生的S期、G2+S期比率与慢性炎症相有显著差异(P<0.01);各型病变与正常胃黏膜相比均有显著差异(P<0.01)。(4)四种胃黏膜病变的Fas表达率由慢性炎症—轻度增生—中、重度增生—胃癌递减;胃癌组与其他四组相比有显著差异(P<0.01及P<0.05)。DI值高者其Fas表达率下降(r=-0.43),二者具有负相关性。结论从正常胃黏膜—慢性萎缩性胃炎—轻度不典型增生—中、重度不典型增生—胃癌过程中,异倍体出现率递增,Fas表达率递减,S期,G2+S期比例递增,DI值增高。DNA倍体分析与Fas表达有相关性。在胃癌的早期诊断、恶性程度的判定、预后估计及治疗上有一定的临床应用价值。
【Abstract】 Objective In order to study the change and correlativity of DNA ploidy analysis and Fas expression in human gastric carcinogenesis,investigate the possible influence of cell proliferation and apoptosis on gastric tumorigenesis and development by the methods of quantitative pathology and clinical immunology.Methods Using flow cytometer and immunofluorescence techni,we observed the DNA ploid and Fas expression in situ in 20 normal gastric mucosa,43 chronic gastritis,35 I dysplasia,26 Ⅱ-Ⅲ desplasia and 38 carcinoma.Results The incidence of aneuploid in Ⅱ-Ⅲ dysplasia and carcinoma were signigicantly higher than in chronic gastritis and I dysplasia(P<0.01).The DI in carcinoma was significantly higher than that in normal gastric mucosa and gastric dysplasia(P<0.01),normal gastric mucosa was significantly lower than another types(P<0.01);I dysplasia was significantly higher than that in chronic gastritis(P<0.05).In diploid specimen,the rate of S and S +G2 in carcinoma and Ⅱ-Ⅲ dysplasia were significantly higher than that in chronic gastritis(P<0.01);normal gastric mucosa were significantly lower than another types(P<0.01).Fas was differently expressed in normal mucosa,precancerous lesions and caroinomas,caroinomas were significantly lower than another types(P<0.05).DI was generally increased and Fas decreased.These findings suggest that correlations between DNA ploid and Fas expression are present(r=-0.43).Conclusion In the process of normal gastric mucosa,chronic gastritis,dysplasia and carcinoma,the incidence of aneuploid,S,S+G2 and DI were increased and Fas decreased.The resuls showed that Fas can supply the defect of DNA analysis,detecting DNA ploid and Fas can play the important role in early diagnosis,treatment,prognosis,recurrence of gastric carcinogenesis.
