【摘要】 目的观察3种不同血液净化方法对维持性血液透析患者血磷及甲状旁腺素(PTH)的清除效果。方法将36例维持性血液析透患者分为3组,分别采用血液透析滤过(HDF)、血液灌流+血液透析(HP+HD)、血液透析(HD)治疗,连续治疗2个月后观察血磷及PTH的变化。结果HDF组治疗后血磷降至(2.12±0.56)mmol/L,PTH降至(378±168)pg/ml;HP+HD组治疗后血磷降至(1.73±0.65)mmol/L,PTH降至(306±157)pg/ml;HD组治疗后血磷降至(1.82±0.61)mmol/L,PTH降至(516±202)pg/ml;3组治疗后血磷及血PTH水平均明显下降(P<0.05)。此外,HDF组及HP+HD组血磷及PTH清除率均明显大于HD组(P<0.05)。结论HDF及HP+HD均能有效清除血磷和PTH,而HD仅能有效清除血磷,不能有效清除PTH。更多还原

【Abstract】 Objective To compare the clearance effect of different blood purification methods on serum phosphorus and parathyroid hormone (PTH) levels in patients on maintenance hemodialysis. Methods Thirty-six patients on maintenance hemodialysis were randomly divided into three groups:hemodiafiltration group(HDF),hemoperfusion plus hemodialysis group(HP+HD), hemodialysis group(HD). Serum phosphorus and serum PTH levels were observed before and after two month-continuing treatment. Results In HDF group serum phosphorus was decreased significantly from(2.68±0.62)mmol/L to (2.12±0.56)mmol/L(P<0.05)and serum PTH was reduced significantly from(760±204)pg/ml to (378±168)pg/ml(P<0.05). In HP+HD group serum phosphorus was decreased significantly from(2.30±0.74)mmol/L to(1.73±0.65)mmol/L(P<0.05)and serum PTH was reduced significantly from(724±192)pg/ml to (306±157)pg/ml(P<0.05). In HD group serum phosphorus was decreased significantly from(2.56±0.85)mmol/L to (1.82±0.61)mmol/L(P<0.05),while serum PTH reduced from (686±210)pg/ml to (516±202)pg/ml(P>0.05). Conclusion Hemodiafiltration or hemoperfusion plus hemodialysis can safely and effectively clear serum phosphorus and PTH. Hemodialysis alone can effectively clear serum phosphorus, but cannot clear PTH effectively.更多还原
