腰椎副突形态的多层螺旋CT三维重建研究
Study on morphology of lumbar accessory process by volume reconstruction of MSCT
【摘要】 [目的]研究腰椎副突MSCT三维重建的形态特征,为临床提供详尽的影像基础资料。[方法]利用MSCT对40例正常成人腰椎进行薄层扫描,三维重建后观察副突。对副突进行形态学分型,测量清晰显示副突所需的旋转和前倾角度、副突高度及乳副突间距,并分析其分布特点。[结果]三维重建图像侧向旋转13.8°±3.75°,轴向前倾26.4°±6.06°后可清晰显示腰椎副突结构。副突可分5型,I型(乳头状)占21.5%(86/400),II型(锥状)占17.2%(69/400),III型(舌状)占14.5%(58/400),IV型(残痕状)占27.0%(108/400),V型(无副突结构)占19.8%(79/400)。V型70%位于L4、5(55/79);各型副突间各指标有显著性差异(P=0.000),性别间各指标无显著性差异(P>0.05)。[结论]MSCT三维重建图像可清晰显示形态各异的腰椎副突,并为临床应用提供影像学基础。
【Abstract】 [Objective] To study the morphologic characteristics of the lumbar accessory processes on volume reconstruction image of multi-slice spiral computed tomography(MSCT),and to provide basic information for clinical application.[Method] Lumbar vertebrae of 40 normal adults were scaned with MSCT,and their accessory processes were observed after volume reconstruction.Accessory processes were classified by morphology,angles of rotation and anteversion were measured.The height and distance between accessory process and mastoid process were measured.Characteristics of their distributions were analyzed.[Result] Lumbar accessory processes could be displayed clearly when the volume reconstruction images were rotated 13.8°±3.75° laterally and anteverted 26.4°±6.06° axially.Lumbar accessory processes could be divided into 5 types: Type I(mammilliform type) 21.5%(86/400),Type II(coniform type) 17.2%(69/400),Type III(liguliform type) 14.5%(58/400),Type IV(incomplete type) 27.0%(108/400),and Type V(none accessory process type) 19.8%(79/400).70%(55/79) of Type V was located on L4 or L5.There was a significant difference in parameters between the five types of accessory processes(P=0.000).No significant difference of parameters between male and female was found(P>0.05).[Conclusion] Lumbar accessory processes could be displayed clearly by MSCT volume reconstruction,which offers great help in the clinical practice.
