肺癌甲状腺转录因子-1的表达及其与细胞凋亡和血管新生关系探讨

【摘要】 目的:探讨肺癌甲状腺转录因子-1(TTF-1)表达与细胞凋亡和血管新生的关系。方法:构建含196例肺癌组织、10例正常肺组织和1例肌肉组织的829点阵的石蜡组织芯片。用原位末端转移酶标记检测法(TUNEL)和免疫组化SP法,检测细胞凋亡及TTF-1和CD34的表达。应用LeicaQ500MC图像分析系统测试TTF-1阳性单位值(Positive Unit,PU)、细胞凋亡指数(Apoptotic Index,AI)和微血管密度(Microvessel Density,MVD)。结果:肺小细胞癌和大细胞癌AI小于肺腺癌和鳞癌,差异有显著性(P=0.000)。有淋巴结转移组AI小于无淋巴结转移组,差异有显著性(P=0.039);TNMⅡ~Ⅳ期组AI小于Ⅰ期组,差异有显著性(P=0.008)。肺小细胞癌TTF-1的PU值与AI呈负相关(r=-0.752,P=0.000)。无淋巴结转移组MVD小于有淋巴结转移组(P=0.031);TNMⅠ期组MVD小于Ⅱ~Ⅳ期组,差异有显著性(P=0.040)。肺腺癌TTF-1的PU值与MVD呈正相关(r=0.708,P=0.000)。结论:肺小细胞癌TTF-1的PU值与AI呈负相关,二者之间可能存在一定相互作用关系。肺腺癌TTF-1的PU值与MVD呈正相关,TTF-1可能通过促进肿瘤微血管生成促进肺腺癌的发生、发展。

【Abstract】 Objective:To investigate the correlation between the expression of thyroid transcription factor-1(TTF-1) and apoptosis and angiogenesis in lung cancer.Methods:A paraffin tissue microarray containing 829 tissue cores including 196 lung carcinomas,10 normal lung tissues and 1 muscular tissue was constructed.Terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL)and immunohistochemical staining were used to detect apoptosis and the expression of TTF-1 and CD34 in different types of lung carcinomas.A Leica Q500 MC image analysis system was employed to measure and calculate TTF-1 Positive Units(PU),Apoptot- ic Index(AI),and Microvessel Density(MVD).Results:AI of small cell lung cancer and large cell lung cancer was lower than that of adenocarcinoma and squamous cell carcinoma of the lung(P=0.000).AI of lung carcino- mas with lymph node metastases was lower than that of those without lymph node metastases(P=0.039).AI of lung carcinoma in TNM stageⅡ-Ⅳwas lower than that in stageⅠ(P=0.008).TTF-1 PU was negatively cor- related with AI in small cell lung cancer(r=-0.752,P=0.000).MVD of lung carcinomas without lymph node metastases was lower than that of those without lymph node metastases(P=0.031).MVD of lung carcinomas in TNM stageⅠwas lower than that in stageⅡ-Ⅳ(P=0.040).TTF-1 PU was positively correlated with MVD in lung adenocarcinoma(r=0.708,P=0.000).Conclusion:There is a negative correlation between TTF-1 PU and AI in small cell lung cancer.TTF-1 PU was positively correlated with MVD in lung adenocarcinoma.TTF-1 may contribute to the development of lung adenocarcinoma by inducing tumor angiogenesis.
