2型糖尿病药物常规治疗与强化治疗的生理指标与主观指标评价

【摘要】 目的:从临床生理指标和病人主观体验指标两个角度,评价2型糖尿病药物强化治疗和常规治疗两种方案的疗效,为糖尿病药物治疗方案的选择提供科学依据。方法:研究对象来自社区病例,临床生理指标采用的是空腹血糖、餐后2小时血糖、糖化血红蛋白;病人主观体验指标采用的是SF-36。结果:经重复测量资料的方差分析,两种方案临床生理指标和生命质量总评分及各维度得分的差异不具有统计学意义,但在时间序列上的差异,除生命质量的精神健康得分外,其他均有统计学意义(P<0.05)。结论:2型糖尿病药物常规治疗和强化治疗相对自身均是有效的,但两种方案之间的疗效差异无统计学意义,即疗效均等;考虑到门诊费用等因素,可将强化治疗作为2型糖尿病药物治疗的首选方案。

【Abstract】 OBJECTIVE:To evaluate the efficacy of intensive insulin therapy(IIT)vs.routine therapy(RT)for non-insulin-dependent diabetes mellitus(NIDDM)by comparing clinical physiology and patients’ subjective experience indexes so as to provide scientific basis in drug choice for NIDDM patients.METHODS:The subjected were enrolled from community patients.Fasting serum glucose(FPG),postprandial serum glucose at 2 h and glycosylated hemoglobin(HbA1c)were used as clinical physiology indexes while SF-36 was adopted as patients’ subjective experience indexes.RESULTS:The ANOVA results for the repeated measurement data showed that there were no significant difference in clinical physiologic indexes,scores of life quality and scores in different dimension between the two therapeutic schemes,however,as for time series,all were significant except there were scores for the emotional health in life quality(P<0.05).CONCLUSION:Both ⅡT and RT are equally effective for NIDDM patients as compared with self control.Considering the outpatient charges and other factors,it is advisable to take ⅡT as the choice drug for NIDDM patients.
