【摘要】 目的应用组织多普勒技术评价代谢综合征左室肥厚患者心房内和心房间的同步性。方法研究对象为22例代谢综合征伴左室肥厚患者(MSLVH组)、69例代谢综合征不伴左室肥厚患者(MSNLVH组)和33例正常人(对照组)。于心尖四腔观,测量左房游离壁、房间隔及右房游离壁取样点处P波起始点至Q-TVI曲线上的A波起始点的时限P-LA、P-IAS、P-RA,左房内同步性定义为P-LA与P-IVS的差值,右房内同步性定义为P-IVS与P-RA的差值,左右心房间的同步性定义为P-LA与P-RA的差值。结果与对照组相比,MSNLVH组和MSLVH组存在显著的左房及心房间的不同步,MSLVH组与MSNLVH组相比,MSLVH组左房及心房间的不同步更加明显。结论代谢综合征患者存在左房及心房间的不同步,左室肥厚是导致代谢综合征患者出现左房内和心房间不同步的重要影响因素。更多还原

【Abstract】 Objective To assess intra-and inter-atrial synchronicity in patients with metabolic syndrome (MS) and left ventricular hypertrophy (LVH) by Tissue Doppler imaging.Methods Twenty-two patients with MS and LVH (MSLVH Group),69 patients with MS and non-LVH (MSNLVH Group) and 33 healthy subjects (Control Group) were included.As to assess atrial synchronicity,myocardial velocity curves were reconstituted off-line using the left atrial (LA) free wall,the inter-atrial septum (IAS),and the right atrial (RA) free wall.The time differences from the onset of the P wave to the onset of the A wave at LA (P-LA),IAS (P-IAS),and RA (P-RA) were measured.Intra-atrial asynchrony was defined as the differences between P-IAS and P-RA (RA asynchrony) and between P-LA and P-IAS (LA asynchrony).Inter-atrial asynchrony was defined as the difference between P-LA and P-RA.Results Compared with the control group,MSNLVH and MSLVH group had significant LA and inter-atrial asynchrony.MSLVH had much more obvious LA and inter-atrial asynchrony when compared with MSNLVH group.Conclusions Patients with MS have LA and inter-atrial asynchrony.LVH is an important factor leading to these asynchronies in the patients with MS and LVH.更多还原
