原发性肝癌超声造影定量血流灌注参数与病理微血管密度对比分析

【摘要】 目的探讨原发性肝癌(HCC)超声造影(CEUS)定量血流灌注参数与肿瘤病理微血管密度(MVD)的相关性。方法HCC34例术前行CEUS,造影后定量分析峰值时间增强图像的平均血流密度(MFD);以TIC声学定量分析软件绘制时间-强度曲线(TIC),得出病灶绝对增强强度(AEI),以上所得参数与术后病理组织免疫组化染色计测的MVD对照,进行相关性分析。结果34例病灶峰值时间计测的MFD为(0.79±0.16);病灶的AEI为(7.44±3.56)dB;术后肿瘤病理MVD为(62.59±23.96);术前病灶MFD与术后病理MVD呈正相关(r=0.85,P<0.01),病灶AEI与MVD无相关关系。结论HCC超声造影峰值MFD与肿瘤MVD相关良好,是定量评价肿瘤血流灌注的可靠依据。

【Abstract】 Objective To investigate the correlation between quantitative parameters with contrast-enhanced ultrasound(CEUS)and microvessel density(MVD)in hepatocellular carcinoma(HCC).Methods Thirty-four patients with HCC underwent CEUS examination before operation.The peak intensity time curves on the enhanced images were analyzed quantitatively with computer to get mean flow density(MFD).TIC quantification software was used to delineate time-intensity curve(TIC),and to count absolute enhanced intensity(AEI).These parameters were compared with MVD counted with immunohistochemistry,and the correlation was statistically studied.Results The MFD for the peak intensity time in the enhanced images was(0.79±0.16),the AEI was(7.44±3.56)dB,and the MVD of HCC after operation was 62.59±23.96 in 34 cases.The MFD before operation was correlated positively to MVD(r=0.85,P<0.01)of histological section after operation.There was no correlation between AEI and MVD in lesions.Conclusions MFD calculated with CEUS was correlated to MVD of tumors with immunohistochemistry.It is a valuable index for quantitative evaluation of tumor blood perfusion.
