C-反应蛋白及纤维蛋白原对急性冠状动脉综合征的影响
Effects of C-Reactive Protein and Fibrinogen to Acute Coronary Syndrome
【摘要】 目的通过观察C-反应蛋白及纤维蛋白原的水平变化,探讨炎症标记物在急性冠状动脉综合征中的关系及临床意义。方法ACS组122例,其中急性心肌梗死亚组46例,不稳定性心绞痛亚组76例,选择稳定性心绞痛66例作对照组。分别采用速率散射比浊法及热沉淀比浊法测定血清CRP及FIB。结果ACS组CRP高于SAP组([18.82±17.32)mg/Lvs(1.96±0.88)mg/L,P<0.05],AMI亚组明显高于UAP亚组与SAP组([28.74±23.19)mg/Lvs(11.03±10.17)mg/L、(1.96±0.88)mg/L,P<0.01],UAP亚组高于SAP组([11.03±10.17)mg/Lvs(1.96±0.88)mg/L,P<0.05]。ACS组FIB高于SAP组([3.55±1.33)g/Lvs(2.46±2.77)g/L,P<0.05],AMI亚组明显高于UAP亚组与SAP组([3.98±1.46)g/Lvs(3.23±1.02)g/L、(2.46±2.77)g/L,P<0.01],UAP亚组高于SAP组([3.23±1.02)g/Lvs(2.46±2.77)g/L,P<0.05]。结论炎症参与了ACS患者的发病过程,冠心病患者血清CRP和FIB水平增高可作为ACS的危险信号,同时也可以作为CHD患者危险分层的重要生化监测指标。
【Abstract】 Objective To investigate the relation of inflammation markers to acute coronary syndrome(ACS) and the clinical significance by observing the changes of C-reactive protein(CRP) and fibrinogen(FIB).Methods The ACS group included 122 patients,which were composed of the acute myocardial infarction(AMI) subgroup(46 patients) and the unstable angina pectoris(UAP) subgroup(76 patients),the control group were 66 patients with stable angina pectoris(SAP).All of them were measured serum CRP and FIB.Results Serum CRP was higher in ACS group compared with SAP group[(18.82±17.32)mg/L vs(1.96±0.88)mg/L,P<0.05],which of AMI subgroup was much higher than UAP subgroup and SAP group[(28.74±23.19)mg/L vs(11.03±10.17)mg/L,(1.96±0.88)mg/L,P<0.01],of UAP subgroup was higher than SAP group[(11.03±10.17)mg/L vs(1.96±0.88)mg/L,P<0.05].FIB was higher in ACS group compared with SAP group[(3.55±1.33)g/L vs(2.46±2.77)g/L,P<0.05],which of AMI subgroup was much higher than UAP subgroup and SAP group[(3.98±1.46)g/L vs(3.23±1.02)g/L,(2.46±2.77)g/L,P<0.01],of UAP subgroup was higher than SAP group[(3.23±1.02)g/L vs(2.46±2.77)g/L,P<0.05].Conclusion Inflammation takes part in the course of ACS.The elevated serum CRP and FIB levels of the patients with coronary heart disease(CHD) can be conducted as dangerous singers in the onset of ACS,and as important biochemical monitoring index in dangerous gradation of the patients with CHD.
