脐带绕颈不同分娩方式的结局分析与对策

【摘要】 目的探讨脐带绕颈的分娩结局与分娩方式的关系。方法将260例脐带绕颈病例分为剖宫产组和阴道分娩组,比较两组的绕颈周数、脐带绕颈的产前知晓率、胎儿窘迫及新生儿窒息发生率等情况,对于B超提示脐带绕颈者,进行严密监测,做好产前监护及健康宣教,并根据脐带绕颈情况采取不同的分娩方式;同期脐带绕颈组(260例)又与非绕颈组(926例)比较。结果绕颈组比较中,剖宫产组的产前知晓率明显比阴道分娩组高(P<0.05),阴道分娩组的胎儿窘迫率比剖宫产组略高,其它各指标无明显差异,而绕颈组的剖宫产率比非绕颈组明显增高(P<0.05)。结论脐带绕颈并不是剖宫产指征,但脐带绕颈可给围产儿带来不良影响,只要对脐带绕颈者加强产时监护,大部分是可经阴道分娩的。更多还原
