【摘要】 目的探讨老年高血压病与高血压病合并2型糖尿病心率变异性(HRV)的特点。方法采用24小时动态心电图监测心率变异及心律失常,比较原发性高血压病(EH)组38例、EH合并2型糖尿病(2DM)组38例与健康体检组40例的HRV特点及早搏发生情况。结果EH组和EH合并2DM组与对照组HRV时域比较所有参数差异均有显著性意义(P<0.001);EH合并2DM组与EH组比较除心搏数的百分比(PNN50)参数差异无显著性意义外(P>0.05),24小时每5分钟的节段窦性R-R间期平均值的标准差(SDANNIndex)、24小时5分钟的节段窦性R-R间期标准差的平均数(SDNNIndex)、连续正常R-R间期差的均方根(RMssd)等参数比较差异均有显著性意义(P<0.001)。EH组及EH合并2DM组的早搏发生率均高于对照组(P<0.001)。结论动态心电图HRV监测可以判断老年EH与EH合并2DM患者心脏植物神经功能损害程度,对疾病的预后评价有重要的临床价值。更多还原

【Abstract】 Objective To investigate the characteristic of heart rate variability(HRV)of elderly patients with hypertension or hypertension combining diabetes.Methods The heart rate variation and arrhythmia were observed with continuously dynamic electrocardiogram monitoring.The HRV and arrhythmia were compared in 38 cases of essential hypertension(EH),38 cases of hypertension with diabetes and 40 healthy subjects.Results There were significant differences among hypertension group,hypertension with diabetes group and control group(P < 0.001).Except PNN50,there were significant differences in the standard deviation of mean value and the mean value of standard deviation in sinus R-R interval at every 5 minutes for continuous 24 hours monitoring,and the mean square root of difference in continuous normal R-R interval(P < 0.001).The frequency of premature beat in EH and EH combining diabetes patients was higher than that in controls(P < 0.001).Conclusion Dynamic electrocardiogram HRV surveillance can identify the damage degree of heart vegetative nerve function in the elderly with hypertension and hypertension associated with diabetes.更多还原
