【摘要】 目的观察低分子肝素钙(LMWH)与坎地沙坦联合治疗慢性肺原性心脏病(CPHD)心力衰竭的疗效及其对心肌重塑的影响。方法将69例住院患者随机分为两组,治疗组(35例)在对照组(34例)常规治疗基础上加LMWH及坎地沙坦治疗,总疗程为6周。于治疗后2周和6周测定并观察临床疗效。结果治疗组总有效率明显高于对照组(P<0.05),治疗组动脉氧分压(PaO2)及二氧化碳分压(PaCO2)较对照组明显改善,右室流出道内径(RVOT)、右心室内径(RV)、肺动脉内径(PA)、左室舒张末期容量(LVED)明显缩小(P<0.05),左室射血分数(LVEF)明显改善(P<0.05)。结论在常规治疗基础上加用LMWH及坎地沙坦可以更好地改善CPHD心力衰竭患者的心功能和临床症状,改善肺血管及右心室重塑,降低肺动脉压。更多还原

【Abstract】 Objective To observe the clinical effects of low molecular weight heparin(LMWH) combined with candesartan for treating CPHD heart failure and its effect on myocardial remodeling.Methods A total of 69 inpatients with CPHD were divided randomly into treatment group(n=35) and control group(n=34).The patients in the treatment group received LMWH and candesartan in addition to conventional therapy of CPHD,whereas the patients in the control group were only given the conventional therapy,both of patients treated for 6 weeks.The clinical signs were observed before and after 2 week,6 week.Results There were obviously improved in PaO2,PaCO2( P < 0.05) ,and RVOT,RV,PA,LVED were shorter,especial in 6 weeks after the treatment.Conclusion On the basis of conventional therapy,combined LHWH and candesartan can markedly relieve the syndromes,improve the cardiac functions,pulmonary vessels and myocardial remodeling,and decrease pulmonary arterial pressure in CPHD heart failure.更多还原
