IgA肾病肾组织乙型肝炎病毒DNA的检测及意义探讨
Fluorescence quantitative PCR detection of HBV-DNA and its implications in IgA nephropathy
【摘要】 目的对血清乙型肝炎病毒(HBV)标志物阴性IgA肾病患者肾组织进行HBV-DNA的检测,为临床诊疗提供理论依据。方法应用原位杂交(ISH)技术检测56例血清HBV抗原阴性和8例血清HBV抗原阳性的肾炎患者肾活检石蜡包埋组织切片中的HBV-DNA。用荧光定量聚合酶链反应(FQ-PCR)对14例血清HBV抗原阴性和8例血清HBV抗原阳性的肾炎患者肾新鲜活检组织中的HBV-DNA。结果ISH在血清HBV抗原阳性的8例标本中HBV-DNA的检出率为88%(7/8),血清HBV抗原阴性56例中检测出HBV-DNA11例(20%)。原位杂交显示两组检出的HBV-DNA均以肾小管上皮细胞胞质分布为主,与组织中HBV抗原阳性颗粒的分布基本一致。荧光PCR在血清HBV抗原阳性的8例标本中HBV-DNA的检出率为100%(8/8),血清HBV抗原阴性14例中检测出HBV-DNA0例(0%)。结论HBV-DNA在IgA肾病肾组织中的检出,表明HBV在IgA肾病的发病机制中可能有着重要作用。
【Abstract】 Objective To detect kidney HBV-DNA in sero-negative patients with IgA nephropathy (IgAN) and to provide theoretical evidences for clinical diagnosis and therapy. Methods HBV-DNA in paraffin-embedded renal samples tissue was detected using in-situ hybridization (ISH) for 8 sero-positive and 56 sero-negative IgAN patients, and using fluorescence quantitative PCR (FQ-PCR) for 8 sero-positive and 14 sero-negative cases. Results HBV- DNA in renal biopsy was detected in 7 of 8 (88%) sero-positive and 11 (20%) sero-negative IgAN patients with ISH, in all (100%) sero-positive and none of 14 (0%) sero-negative IgAN patient with FQ-PCR. By ISH, HBV-DNA was localized predominantly in the cytoplasm of tubular epithelia, which was consistent with the distribution of HBV-Ag. Conclusion Presence of renal HBV-DNA in IgAN patients suggests an important role of HBV in development of this disease.
