三维MR涎管造影在涎腺疾病中的应用

【摘要】 目的探讨三维磁共振(MR)涎管造影在常见涎腺病变中的应用价值。方法应用1.5 T超导型MR机对30例涎腺病变患者进行MR涎管造影检查,对相应影像特点进行观察和总结,对其临床价值进行评价。结果3例急性颌下腺炎患者的MR涎管造影见涎管轻度扩张。慢性涎腺炎患者的末梢导管点球状扩张,主导管可不同程度扩张,并可见扩张与狭窄并存的腊肠样改变。涎石症患者的涎管内可见无信号结石影或杯口状充盈缺损,结石近侧涎管往往有扩张。舍格伦综合征患者的MR涎管造影中可看到双侧末梢导管广泛点球状扩张。6例涎腺区良性肿瘤患者的涎管呈受压移位改变。1例腮腺癌的MR涎管造影显示腺体内导管破坏、移位。结论三维MR涎管造影可清晰无创地显示常见涎腺疾病的涎管和腺泡情况,有望取代X线涎管造影而成为首选的影像学检查方法。

【Abstract】 Objective To analyze the changes of the disorder of salivary gland ducts in common salivary gland diseases with MR sialography,and evaluate the diagnostic value of MR sialography.Methods 3D heavily T2-weighted fast spin echo sequence MR sialography was performed in 30 patients with different salivary gland diseases.All images were observed and summarized.Results In 3 cases with acute submaxillaritis,the main duct became slightly wider.In patients with chronic sialadenitis,the dilatation of main ducts to different degrees could be seen,so did the dilated terminal ducts and sausagelike change of main ducts.In patients with sialolithiasis,filling defects were showed in salivary ducts that corresponded to sialoliths,customarily with dilatation of the proximal ducts.In patients with Sjo gren syndrome,a punctate,globular,cavitary,or destructive appearance was well seen within the parotid glands.In 6 cases with benign tumor,the ducts were displaced.The duct in one patient with parotid gland cancer showed displacement and destruction.Conclusion 3D MR sialography is noninvasive and allows delineation of both normal and abnormal parotid and submandibular gland duct systems.MR sialography combined with conventional MR imaging has the potential to replace conventional sialography.
