时间-温度阈值控制肝脏激光间质热消融治疗的实验研究
Experinental study on time-temperature thresholds-controlled laser-induced interstitial thermotherapy in liver
【摘要】 目的:探讨应用时间-温度阈值控制肝脏LITT的可行性。方法:16只实验兔,分成2组,均在介入性MRI引导下行LITT。组1和组2分别加热到60℃和时间-温度阈值。测量光导纤维尖端至测温针尖的距离(D1),以及光导纤维尖端至凝固性坏死最外缘的距离(D2)。结果:阈值温度54℃最为多见。组1D1与D2的差异有显著性统计学意义。组1达到时间-温度阈值平均节约时间(或能量)26.63%。结论:时间-温度阈值控制肝脏LITT能完全消融肝组织,节约消融时间和能量,减少正常组织损伤。
【Abstract】 Objective:To evaluate the feasibility of using time-temperature thresholds-controlled LITT in liver.Methods:Sixteen New Zealand White rabbits(mean 3.69 kg) were divided into 2 groups randomly,underwent interventional MRI-guided LITT by laser applications(Diamed,UK).The group 1 stopped ablation until the temperature reached 60℃;and the group 2 stopped ablation as soon as the time-temperature thresholds achieved.The distance(D1) from the tip of optical fiber to the tip of probe was measured on the plane consisted of the long axes of needle and probe,and the distance(D2) from the tip of optical fiber to the outermost margin of the coagulated necrosis was measured along the above measurement line.Results:All samples of 2 groups(16/16) demonstrated coagulated necrosis of hepatic tissue surrounding the tip of probe.The most common threshold temperature was 54℃(6/16,37.5%).In group 1,the differenoe between D1 and D2 was statistically significant(Z=-2.201,P=0.028,P<0.05).The mean percentage of saved time(or energy) was 26.63%.Conclusion:Time-temperature thresholds-controlled LITT can completely ablate hepatic tissue,save time and energy of ablation,decrease the injury of normal tissue,and increase the expected size of ablation.
