不稳定型心绞痛患者幽门螺杆菌抗体水平与糖代谢异常关系的探讨

【摘要】 目的观察不稳定型心绞痛(UAP)患者血清幽门螺杆菌(Hp)IgG水平与糖代谢异常的关系。方法UAP患者114例,根据口服葡萄糖耐量试验(OGTT)分为糖代谢正常组(29例)和糖代谢异常组(包括IGT和DM,85例),根据胰岛素抵抗指数(HOMA-IR)分为非胰岛素抵抗组(82例)和胰岛素抵抗组(32例);稳定型心绞痛(SAP)患者38例。所有患者应用酶联免疫吸附实验(ELISA)法测定血清幽门螺杆菌IgG抗体(Hp-IgG)浓度,测定血清hsCRP,行OGTT试验,采用稳态模型分析法计算HOMA-IR,行选择性冠状动脉造影检查,以Gensini冠状动脉积分评价冠状动脉狭窄程度。结果①UAP组的血清Hp-IgG、hsCRP、空腹血糖(FPG)和冠状动脉积分高于SAP组。②糖代谢异常组的FPG、HbA1c(P<0.01)和冠状动脉积分(P<0.05)高于糖代谢正常组。胰岛素抵抗组的Hp-IgG水平和非胰岛素抵抗组无差别,糖代谢异常组和糖代谢正常组的血清Hp-IgG水平差异也无统计学意义(均P>0.05)。结论UAP组的血清Hp-IgG水平高于SAP组,Hp感染可能参与了UAP的发病,而这种作用与糖代谢异常无关。

【Abstract】 Objective To observe the relationship between the serum levels of Ig-G antibodies of helicobacter pylori (Hp) of the patients with unstable angina pectoris (UAP) and abnormal glucose metabolism. Methods One hundred and fourteen patients with UAP were recruited consecutively as UAP group and they were divided into 2 groups according to results of oral glucose tolerance tests (OGTT):29 patients with normal glucose tolerance(NGT).85 patients with abnormal glucose metabolism[include impaired glucose tolerance(IGT)and type 2 diabetes mellitus(DM)]. There were 38 patients with stable angina pectoris(SAP)in control group.The concen- tration of Hp-IgG were determined by the enzyme linked immunosorbent assay (ELISA). The concentration of the hsCRP were determined. The OGTT were performed by chemoluminescence technique. The degree of insulin resis- tance was assessed using insulin resistance index by homeostasis model assessment(HOMA IR).Coronary angiography was performed in all patients to assess the degree of coronary artery stenoses. Results The serum levels of Hp-IgG,hsCRP,fasting plasma glucose (FPG)and Gensini’s scores were higher in UAP group than in the SAP group. The levels of FPG,HbA1c and the degree of coronary artery stenoses were higher in the UAP patients with abnormal glucose metabolism than those in the UAP patients with NGT. The serum levels of Hp-IgG was no sig- nificant difference between the UAP patients with insulin resistance and with non-insulin resistance.The serum lev- els of Hp-IgG was no significant difference between the UAP patients with abnormal glucose metabolism and with NGT. Conclusion The serum levels of Hp-IgG is higher in UAP group than in the SAP group.Hp infection may participate in development of UAP,which role independent of abnormal glucose metabolism.
