【摘要】 目的探讨甲状腺相关眼病(thyroid-associated ophthalmopathy,TAO)的眼外肌病变的治疗方法及疗效。方法回顾分析自2001年1月至2007年10月来我院就诊的21例23眼TAO致眼外肌病变患者,通过对不同时期患者分别实施保守治疗及手术治疗,观察治疗前后患者的眼位、斜视度等情况。结果17眼活动期患者经保守治疗后,眼部水肿、复视、斜视有明显改善。6眼静止期患者,其中4眼行下直肌、内直肌后退术,2眼行下直肌断腱术,2眼治愈,4眼明显改善。眼位欠矫4眼。结论TAO以多条直肌受累为主,对甲状腺相关眼病不同时期分别采取不同的治疗方案,都可以得到良好的临床效果。更多还原

【Abstract】 Objective To investigate the management of extraocular myopathy caused by thyroid-associated ophthalmopathy and value its curative efficacy.Methods Twenty-one cases(23 eyes) of extraocular myopathy caused by thyroid-associated ophthalmopathy, which were diagnosed at our hospital from January,2001 to October,2007, were analyzed retrospectively. Patients at different disease stages were treated with different methods ranging from conservative treatment to surgery. Eye position and strabismus angle were evaluated before and after the treatment.Results Symptoms like eyelids edema, diplopia and strabismus improved a lot after conservative treatment in 17 eyes at active stage. Of the 6 eyes at resting stage, 4 eyes received inferior and medial rectus recession, and 2 eyes received inferior rectus tenotomy;Two eyes were completely cured, and 4 eyes improved a lot. Undercorrection happened to 4 eyes after surgery.Conclusion Thyroid-associated ophthalmopathy often involves in the rectus muscle. With different approaches to patients at different disease stages, effective clinical outcome can be achieved.更多还原
