SGB对创伤性脑损伤患者早期脑血流循环的影响
Effect of stellate ganglion block on cerebral blood circulation in patients with traumatic brain injury
【摘要】 目的利用经颅多普勒超声(TCD)技术和近红外线光谱(NIRS)技术观察星状神经节阻滞(SGB)对创伤性颅脑损伤(TBI)患者早期血液循环的影响。方法随机将20名TBI病人分为对照组和SGB治疗组各10例,对照组接受常规治疗,SGB治疗组接受常规治疗且在明确诊断后即行SGB。对两组患者用NIRS仪持续床旁监测局部脑氧饱和度(rSc02),并在相应时点对两组患者行TCD检查。结果对照组患者伤后第4、7天,大脑中动脉(MCA)平均血流速度(Vm)升高,rScO2降低;与对照组相比,SGB治疗组(Vm)伤后无明显变化,rScO2在SGB后20~30 min开始改善,伤后第2、4、7天均高于对照组。结论SGB治疗可减轻TBI患者早期脑血液循环障碍,提高脑血流量,改善脑组织氧供。
【Abstract】 Objective To evaluate the influence of stellate ganglion block(SGB) on cerebral blood circulation in patients with traumatic brain injury(TBI) using near infrared spectroscopy(NIRS) and transcranial Doppler(TCD) methods.Methods Twenty patients suffering from TBI were divided into control group and SGB group randomly.Patients in control group were treated with conventional symptomatic therapy;those in SGB group underwent SGB as soon as the diagnosis was definite in addition to conventional symptomatic therapy.rScO2 was monitored consistently for all patients with the device of NIRS and middle cerebral artery(MCA) flow velocity at corresponding time using TCD methods.Results Middle cerebral artery Vm increased in control group on day-4 and 7 compared to day-2,reaching its peak on day-7.In SGB group,middle cerebral artery Vm did not show significant changes from day 2 to day 7.rScO2 began to arise within 20-30 minutes after SGB;furthermore,they were higher than control group on day-2,4,7 respectively.Conclusion SGB may have therapeutic value in relieving cerebral vasospasm,improving cerebral blood circulation and resulting in an increase in oxygen supply in the cerebral superficial and deep cerebral regions during early period of TBI.
