三氧化二砷对鼻咽癌放射增敏的近期疗效

【摘要】 [目的]探讨三氧化二砷对鼻咽癌放射增敏作用。[方法]87例T1~4N0~1期鼻咽癌病人随机分为单纯放疗组(对照组)和三氧化二砷+放射治疗组(试验组),观察两组病人鼻咽肿瘤和颈部肿瘤消退情况,1、2年生存率及毒副反应。[结果]放射治疗40Gy时试验组和对照组的鼻咽肿瘤消退率分别为45.45%(20/44)和18.60%(8/43),差异有统计学意义(χ2=7.183,P=0.007)。两组的颈部肿瘤消退速度无统计学差异。40Gy时肿瘤消失组和残留组1、2年无病生存率分别为100%、90.9%和82.2%、69.3%,差异有统计学意义。试验组白细胞降低和恶心呕吐较对照组发生率高,但多为Ⅰ~Ⅱ度反应。其他毒副反应两组间差异无统计学意义。[结论]放疗联合三氧化二砷增敏使鼻咽肿瘤消退更快,且无严重毒副作用。

【Abstract】 [Purpose] To evaluate the radiosensitizing effect of arsenic trioxide(ATO) on nasopha-ryngeal carcinoma(NPC).[Methods] Eighty-seven patients with NPC T1~4N0~1M0 were randomized into two groups:conventional fractionation radiotherapy group(control group) and conventional fractionation radiotherapy plus ATO group(study group).The regressions of nasopharyngeal lesions and cervical lymph nodes,as well as the 1-year and 2-year survival rates and acute toxicities were compared between two groups.[Results] When irradiation dose reached 40Gy,the completely re-gression rates of primary nasopharyngeal lesion were 18.60%(8/43)in control group and 45.45%(20/44) in study group with statistical significance(χ2=7.183,P=0.007).The 1-year and 2-year sur-vival rate was higher in patients with CR than that in those with residual disease at the time of 40Gy delivered(100% vs 82.2% and 90.9% vs 69.3%) with statistical significance(χ2=4.481,P= 0.034).Toxicities in the two groups were not different except of nausea,vomiting and granulocy-topenia,those were more frequent in study group.Most patients experienced grade 1 to 2 acute toxicities in both groups.[Conclusion] ATO combined with radiotherapy may accelerate tumor re-gression in nasopharyngeal carcinoma without severe toxicities.
