BiPAP呼吸机治疗AECOPD合并Ⅱ型呼吸衰竭临床价值探讨
ANALYSIS OF CLINICAL VALUE OF THE BILEVEL POSITIVE AIRWAY PRESSURE (BIPAP) IN PATIENTS WITH ACUTE EXACERBATION OF CHRONIC OBSTRUCTIVE PULMONARY DISEASE (AECOPD) COMBINED WITH TYPE Ⅱ RESPIRATORY FAILURE
【摘要】 [目的]探讨BiPAP呼吸机在治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并Ⅱ型呼吸衰竭中的临床应用价值。[方法]选择42例确诊为COPD急性加重期合并Ⅱ型呼吸衰竭的患者,随机分为对照组和治疗组,对照组给予抗感染、祛痰、平喘等常规治疗,辅以持续低流量鼻导管吸氧;治疗组在常规治疗的基础上,给予BiPAP呼吸机辅助呼吸,每天达12h以上。治疗前及治疗72h后分别查血气分析,观察pH、PaCO2、PaO2、SaO2的变化,记录住院天数,住院费用。[结果]治疗组与对照组相比,治疗组血气分析改善明显,临床症状缓解快,住院天数明显缩短。两组相比差异有统计学意义(P﹤0.05)。[结论]AECOPD合并Ⅱ型呼衰的患者,采用BiPAP呼吸机辅助呼吸,可明显改善机体缺氧及酸中毒,很快缓解临床症状,并且能明显缩短患者的住院天数,减轻患者的经济负担。
【Abstract】 [Objective] To investigate the clinical application value of the bilevel positive airway pressure (BiPAP) in patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD) combined with type Ⅱ respiratory fail-ure. [Methods] 42 patients of AECOPD with type Ⅱ respiratory failure were randomly divided into two groups: control group and treatment group. The control group received the routine therapy including anti-infection, eliminating phlegm, wheeze re-mission assistant with low -flow oxygen therapy by endonasal catheter continually; and the treatment group received BiPAP breathing machine assisted ventilation holding on at least 12 hours and the routine therapy. The measures of arterial gas analysis, including pH, PaCO2, PaO2 and SaO2 were observed before and after therapy 72 hours, meanwhile, the hospital days and ex-pense were counted. [ Results] Compared with control group, arterial gas analysis was significantly improved in treatment group. The clinical symptoms relieved rapidly, and the hospital days reduced in treatment group. There was significant difference between the two groups (P﹤0.05). [Conclusion] The organism hypoxia and acidosis are significantly improved in the patients of AECOPD with type Ⅱ respiratory failure who are treated with BiPAP assisted ventilation, the clinical symptoms are alleviat-ed, the hospital days and the financial burden of the patients are alleviated.
