早期复极综合征的临床意义探讨

【摘要】 目的评价早期复极综合征的心电图特点及临床意义。方法观察住院患者组(患者组)517例和健康体检组(体检组)38例早期复极综合征的心电图和临床资料,其中两组各20例作运动负荷试验。结果(1)本组555例ST段均呈凹面向上或上斜形抬高的典型改变,487例见于单纯胸导联或合并肢体导联(87.74%),绝大多数见于V4~V6。(2)患者组散见于临床近百余种疾病,其分布比例及心电图改变在心血管病与其它系统疾病的差异均无显著性意义(P>0.05);心前区痛待查的28例确诊为合并肺胸膜疾病10例,冠心病、肋软骨炎和神经紧张症各6例。(3)心前区痛待查的20例运动平板试验阳性4例、ST段降至正常16例;体检组20例下蹲运动试验ST段均降至正常。休息10min后39例回复早期复极综合征心电图改变。结论早期复极综合征的心电图特点不具有指证心血管病变的临床意义,不宜再作分型;无心前区痛的早期复极综合征心电图改变为正常变异,无需临床干预。运动负荷试验有助于早期复极综合征的诊断及鉴别诊断。

【Abstract】 Objective To evaluate characteristics of electrocardiogram and clinical significance of early repolarization syndrome(ERS). Methods Electrocardiogram and clinical data were analyzed in 517 inpatients and 38 healthy people with ERS. Results 1. ST segment elevation was concave upward or slope upward in all 555 cases and appeared in precordial leads merely or together with limb leads in 487 cases. Most of them were seen in leads V4 to V6. 2. ERS was seen in different diseases and there was no difference of distribution between cardiovascular and non-cardiovascular diseases in inpatients (P>0.05). 3. 20 inpatients with precordial pain underwent treadmill exercise test. Of them, 4 cases had positive results and another 16 cases had ST segment down to normal. ST segment returned to normal during and elevated again 10 minutes after repeat squatting exercise in all 20 healthy people. Conclusion Characteristic of electrocardiogram in ERS could neither prove cardiovascular diseases nor be subtyped. ERS without precordial pain were normal variation and needed no clinical intervention. Exercise stress test helped to diagnosis and differential diagnosis of ERS.
