【摘要】 目的总结不同椎管入路在后路脊柱内窥镜手术中的应用效果。方法回顾性分析445例后路脊柱显微内窥镜下腰椎间盘摘除患者的临床资料。其中,椎板间隙入路21例,椎板间开窗72例,侧隐窝入路233例,扩大开窗法39例,单侧多平面开窗法和双侧开窗法51例,半椎板切除29例。结果手术均获成功。最常见的并发症是术后患肢有短期麻木感,其中7例(33.3%)为椎板间隙入路,11例(15.3%)为椎板间开窗组。402例随访3~37个月,优305例,良78例,可13例;差6例,均为复发需再次手术治疗。结论各种椎管入路有其不同适应证,应根据不同的病理改变和所需要的暴露范围而选择。更多还原

【Abstract】 Objective To investigate the applicated effect of different approaches to vertebral canal under posterior spinal endoscope.Methods 445 lumbar disc protrusion cases underwent posterior spinal microendoscopic discectomy were reviewed.The approaches were interlaminar approach in 21 cases,intervertebral fenestration in 72,lateral recess approach in 233,enlarged fenestration in 39,unilateral multi-plane and bilateral fenestration in 51,and semi-laminectomy in 29.Results All operations succeeded.18 cases occurred the most encountered complication of short-term numbness in the suffered lower limb,including 7 of 21(33.3%) in the interlaminar approach group and 11 of 72(15.3%) in the intervertebra fenestration group.402 cases were followed up for 3 to 37 months,and the efficacy evaluation revealed excellence in 305 cases,good in 78 cases,fair in 13 cases,and poor in 6 recurrence cases.Conclusion Each approach has its own indications,and should be selected according to pathological changes and necessary exposure extent.更多还原
