微型种植体支抗正畸过程中的稳定性

【摘要】 背景:微型种植体作为绝对支抗应用于正畸治疗中存在的最大问题是稳定性。目的:应用微型种植体支抗内收上颌前牙,观察上颌前牙及第一磨牙位置的变化,评价微型种植体作为强支抗在正畸治疗过程中的稳定性。设计、时间及地点:对比观察实验。于2006-10/2007-12选择河北医科大学第二医院口腔正畸科门诊患者。对象:选取门诊就诊的双颌前突或上颌前突患者20例,男6例,女14例;年龄15~27岁,平均20.7岁。矫治设计拔除4颗第1双尖牙或上颌第1及下颌第2双尖牙。方法:随机选择10例应用上颌后牙区微螺钉型种植体支抗内收前牙,微型钛钉直径为1.5mm,长度为8mm,由西安中邦钛生物制品有限公司提供。所有种植体由同一术者采取相同的手术方式植入。另10例采用腭杆加口外弓的支抗方法。主要观察指标:治疗前及关闭拔牙间隙后拍摄X射线头颅侧位片,测量前牙内收情况、软组织变化及磨牙支抗的变化。结果:①微型钛钉种植体在拔牙间隙关闭的过程中保持了稳定,成功率89%。②种植体支抗治疗后上中切牙倾角及凸距的减少量明显多于非种植体强支抗治疗后的减少量,差异有显著性意义(P<0.01);种植体支抗组磨牙位移明显少于非种植体强支抗组,差异有显著性意义(P<0.01)。结论:微型钛钉种植体作为稳定的骨性正畸支抗,可以实现理想的支抗控制效果。

【Abstract】 BACKGROUND: Stability is the most important problem for orthodontic anchorage. OBJECTIVE: To evaluate the clinical results of mini-implants as skeletal anchorage by investigating the position changes of upper molars and incisors with the application of microscrew implant anchorage during orthodontic treatment for anterior teeth retraction. DESIGN, TIME AND SETTING: Comparative and observational trial was performed at Department of Orthodontics, Second Hospital of Hebei Medical University from October 2006 to December 2007. PARTICIPANTS: Twenty cases of severe maxillary or bimaxillary protrusion, including 6 males, 14 females with an average of 20.7 years (range 15 to 27 years) were selected. Four first premolars or 2 upper first molars and 2 lower premolars were removed. METHODS: The anterior teeth of 10 randomly selected cases were retraced by anchorage of miniscrows (Xi’an Zhongbang) with diameter of 1.5 mm and a length of 8 mm. All surgeries were performed by the same surgeon with the same standard technique. Traditional orthodontic anchorage of transpalatal arches and extraoral headgears were used for the other 10 cases. MAIN OUTCOME MEASURES: Cephalometric lateral measurements before and after the treatment were used to detect anterior teeth retraction, soft tissue and molar changes. RESULTS: The mini-implants remained stable during retracting anterior teeth. The overall success rate of micro-screw implant was 89%. There were significant decreases in the positions and degrees of upper incisors and significant decreases in the displacement of molars for micro-implant anchorage than for traditional orthodontic anchorage during the treatment (P < 0.01). CONCLUTION: Micro-screw implant functioning as stable orthodontic anchorage provides good skeletal anchorage control.
