【摘要】 目的探讨三尖瓣环M型运动曲线在评价右室功能中的价值。方法对36例肺动脉高压患者及20例正常者进行右心功能评价。根据有无右心衰竭临床表现将肺动脉高压组分为两组:肺动脉高压A组(无右心衰组)和B组(右心衰组)。取标准心尖四腔心切面,调节探头以获得最大右室,将取样点置于三尖瓣环侧壁处,每个心动周期测定三尖瓣环从舒末至收末最大的距离。三尖瓣反流程度及速度、心包积液;测定右室面积改变分数。结果肺动脉高压B组三尖瓣环运动幅度和右室面积改变分数(RVFAC)显著低于肺动脉高压A组及正常组。结论肺动脉高压右心衰组三尖瓣环M型运动曲线位移显著降低,可用于评价右室功能。更多还原

【Abstract】 Objective To investigate the right ventricular function by M-mode tricuspid annular plane systolic excursion. Meth-ods The subjects including 36 patients with pulmonary hypertension and 20 volunteers were studied. 36 patients were divided into PHA group (without RV failure) and PHB group (with RV failure). The 4 CH view was used to obtain the largest RV volume by adjusting transducer. An M-mode cursor was steered through the lateral portion of the tricuspid annulus,which delineates the RV atrioventricular plane. The magnitude of tricuspid valve ring along the M-mode line was determined by measuring the maximal distance between the leading edge of the highest and lowest echoes during each cardiac cycle. The velocity and severity of tricuspid regurgitation,pericardial effusion (PE),and the RV fractional area change were measured. Results The TAPSE and RVFAC in PHB group were significantly lower than those in PHA and normal group. Conclusion The significant decrease of TAPSE in PHB group can be used for evaluation of RV func-tion.更多还原
