18F-FDG符合线路显像在食管癌术前淋巴结及远处转移分期中的价值

【摘要】 目的探讨18F-FDG符合线路显像在食管癌淋巴结及远处转移分期中的价值。方法45例食管癌患者行18F-FDG符合线路显像,并与同期CT及术后病理作比较。结果18F-FDG符合线路显像对食管癌淋巴结转移的诊断特异性及阳性预测值优于CT,对食管癌周围组淋巴结诊断特异性及食管癌非肿瘤周围组淋巴结转移诊断特异性、阴性预测值及准确性优于CT。18F-FDG符合线路显像发现7例远处转移。结论18F-FDG符合线路显像对食管癌淋巴结转移判断有一定临床价值,对食管癌的术前分期、治疗方案的制订有重要的临床应用价值。

【Abstract】 Objective To assess the value of 18F-fluorodeoxyglucose(FDG) coincidence SPECT in lymph node and distant metastasis staging of patients with esophageal cancer.Methods 18F-FDG coincidence SPECT and CT were performed and interpreted before surgery in 45 patients with newly diagnosed esophageal cancer.The results were then compared with the associated post surgical histological data.Results The specificity and positive prediction rate of 18F-FDG coincidence SPECT in the detection of lymph nodes metastasis were higher than that in CT detection(P<0.05).18F-FDG coincidence SPECT,the specificity,positive prediction and accuracy in the detection of non-peritumoral lymph nodes,were higher than those in CT detection(P<0.05).The specificity of 18F-FDG coincidence SPECT in the detection of peritumoral lymph nodes metastasis was higher than that in CT detection(P<0.05).18F-FDG coincidence SPECT found distant metastasis in 7 patients.Conclusion 18F-FDG coincidence SPECT can be used as an important tool for the initial staging and selection of treatment planning in the patients with esophageal cancer.
