测定巨细胞病毒特异性抗体亲和指数的诊断价值

Value of determination of specific cytomegalovirus-IgG (CMV-IgG) antibody′s avidity index in the diagnostic of CMV primary infection

【摘要】 目的以快速、简便的人巨细胞病毒特异性抗体亲和指数(CMV-IgG AI)检测方法作为有效的诊断工具,用于区分孕妇CMV原发感染与非原发感染。方法以标准的间接ELISA方法,8.0 mol/L尿素作为温和蛋白变性剂,用标准曲线方法定量测定标本中的CMV-IgG含量。结果共检测218例标本,其中187例为CMV既往感染标本,31例为继发或再感染、可疑原发感染和原发感染标本。以抗体亲和指数<35.0%作为阳性判断标准,此时方法的灵敏度为75.0%,特异度为91.4%,可用度为67.2%。既往感染、继发或再感染、可疑原发感染、原发感染4组的平均AI值分别是(61.42±25.69)%,(70.38±21.88)%,(66.73±23.62)%,(29.78±25.93)%,原发感染组与其它3组非原发感染比较,差异有统计学意义(P<0.01))。结论以8.0 mol/L尿素作为温和蛋白变性剂,用标准曲线方法定量测定标本中CMV-IgG含量适用于检测巨细胞病毒IgG抗体亲和指数(CMV-IgG AI),认为CMV-IgM联合CMV-IgG AI检测是区分原发与非原发感染的血清学方法之一。

【Abstract】 Objective To establish a rapid,easy and simple method for determination of CMV-IgG AI to serve as effective diagnostic tool to distinguish primary CMV infection from non-primary infection in pregnant women.Methods The measurement of CMV-IgG AI is accomplished by standard indirect ELISA method using 8 mol/L urea as a mild protein denaturing agent and the amount of CMV-IgG in the specimen was measured quantitatively using a standard curve method.Results Of the 218 samples,187 were from women with past CMV infection and 31 from those with secondary infection or re-infection,suspicious primary infection and primary infection.When the CMV-IgG AI less than 35.0 % was defined as positive,the sensitivity was 75.0 %,the specificity was 91.4 %,and availability was 67.2 %.The mean AI in the samples from women with past infection,secondary infection or re-infection,suspicious primary infection and primary infection was(61.42±25.69)%,(70.38±21.88)%,(66.73±23.62)%,(29.78±25.93)%,respectively.There is a statistical difference between the primary and non-primary CMV infection(P<0.01).Conclusion The amount of CMV-IgG in the specimen quantitatively measured by a standard curve with 8 m urea solution as a mild protein denaturing agent,was found to be suitable for determination CMV-IgG AI.At present the combination of CMV-IgG AI and CMV-IgM assay seem to be one of the best serological tests for differentiating primary CMV infection from non-primary CMV infection.
