利用组织微阵列研究ACVRL1在鼻咽癌中的表达及临床意义

【摘要】 ACVRL1基因又名活化素受体样激酶1(activin receptor-like kinase-1,ALK1),与2型遗传性出血性毛细血管扩张性疾病(HHT2)密切相关.通过制作含896个组织点阵的鼻咽癌组织微阵列,结合原位杂交法检测ACVRL1在鼻咽癌组织中的mRNA表达,建立ACVRL1基因在鼻咽癌组织中的原位表达谱,分析ACVRL1mRNA的表达与鼻咽癌临床病理特征关系,并分析其与鼻咽癌颈部淋巴结转移的关系.结果发现ACVRL1mRNA在鼻咽癌组织中的表达与相应非癌组织相比,有显著性差异(P<0.05);ACVRL1mRNA的表达与临床分期无显著相关性(P>0.05);与颈部淋巴结有无转移存在显著性差异(P<0.05);但与EBV VCA-IgA血清滴度没有显著相关性(P>0.05).灵敏度、特异度、阳性预测值和阴性预测值等指标综合评估ACVRL1基因作为鼻咽癌颈部淋巴结转移分子标志物的有效性.这些结果表明ACVRL1基因在鼻咽癌组织中表达下调,与颈部淋巴结转移密切相关,可作为鼻咽癌颈部淋巴结是否转移的候选分子标志物.

【Abstract】 ACVRL1(activin receptor-like kinase-1, ALK1) has been identified to be associated with HHT2. NPC tissue microarray including 896 spot arrays combined with in situ hybridization was used to detect the expression of ACVRL1 mRNA in NPC tissues. The in situ expression profile of ACVRL1 mRNA in nasopharyngeal carcinoma (NPC) tissues and analyzed the correlation between ACVRL1 mRNA expression and clinicopathological features and NPC cervix lymph node metastasis were constructed. Results suggested that the expression of ACVRL1 mRNA in NPC was significantly lower than that in their non-cancer tissues (P < 0.05). Expression level of ACVRL1 mRNA had no significant correlation with their clinical stages, EBV VCA-IgA titer( P > 0.05), however it is closely associated with cervix lymph node metastasis of NPC (P < 0.05). Predict values such as sensitivity, specificity, negative predictive values and positive predictive values were comprehensively hired to evaluate ACVRL1 as molecular marker for NPC cervix lymph node metastasis. These results showed that the expression of ACVRL1 mRNA was down-regulated in the NPC tissues and was closely associated with NPC cervix lymph node metastasis. ACVRL1 gene could be used as candidate molecular marker for NPC cervix lymph node metastasis.
