高脂血和高白细胞因素对血红蛋白测定的干扰校正分析
Analysis of rectifying the interferences of hyperlipoidemia and hyperleukocytosis to the measurement of hemoglobin
【摘要】 目的寻求一种校正高脂血和高白细胞(WBC)因素干扰血红蛋白(Hb)测定的方法。方法对38例高脂血和34例高WBC标本分别进行全血细胞分析,采取低速离心并分别用等量生理盐水和仪器配套的稀释液替代上层浑浊血浆,混匀后再进行Hb测定,同时根据文献报道的Hb估算公式估算Hb数值。结果高脂血症组直接测定的Hb值、平均红细胞(RBC)Hb含量(MCH)值、平均RBC Hb浓度(MCHC)值均明显高于生理盐水组、稀释液组和估算组(F=84.92~86.74,q=18.11~19.01,P均<0.001)。高WBC组直接测定的Hb值、MCH值、MCHC值均明显高于生理盐水组、稀释液组和估算组(F=93.81~140.22,q=18.63~24.52,P均<0.001)。在高WBC组,生理盐水组和稀释液组测得的WBC值明显低于直接测定组(F=990.67,q=54.70,54.33,P均<0.001)。结论用等量生理盐水或仪器配套稀释液替代高脂血和高WBC血标本的浑浊血浆,可有效消除浑浊因素对Hb测定的影响,可对Hb进行校正。
【Abstract】 Objective To seek a valid method for rectifying the interferences of hyperlipoidemia and hyperleukocytosis to the determination of hemoglobin(Hb).Methods Complete blood count analysis of 38 hyperlipoidemia cases and 34 hyperleukocytosis cases were carried out,the blood samples were centrifuged by low speed and the cloudy blood plasma was replaced by equal volume of normal saline(NS) and diluent solution of the instrument respectively.After mixture,the Hb was detected and the Hb results were estimated according to the formula reported.Results The Hb,mean corpuscular hemoglobin(MCH) and mean corpuscular hemoglobin concentration(MCHC) of hyperlipoidemia samples determined directly were significantly higher than other groups(F=84.92-86.74,q=18.11-19.01,P<0.001).The Hb,MCH and MCHC of hyperleukocytosis samples determined directly were also significantly higher than other groups(F=93.81-140.22,q=18.63-24.52,P<0.001).There were no significant difference between NS group and dilution group in both hyperlipoidemia and hyperleukocytosis groups.In hyperleukocytosis group,the results of white blood cell(WBC) in NS group and dilution group were significantly lower than the direct determination group(F=990.67,q=54.70,54.33,P<0.001).Conclusions Replacing the cloudy blood plasma with equal volume of NS or dilutent solution can eliminate the interference of hyperlipoidemia and hyperleukocytosis to the measurement of Hb.
