【摘要】 目的通过检测多种血清肿瘤标志物(TM)筛选出对原发性肝癌(PHC)有价值的合理的TM组合方法采用电化学发光法、免疫放射法和连续监测法检测了50例PHC患者、40例肝硬化患者及60名健康人血清中甲胎蛋白(AFP)、α-岩藻糖苷酶(AFU)、癌胚抗原(CEA)、糖链抗原199(CA199)、糖链抗原125(CA125)、糖链抗原50(CA50)、γ-谷氨酰基转移酶(GGT)、碱性磷酸酶(ALP)8种TM水平,对各种TM组合进行平行试验及系列试验分析。结果平行试验敏感性达到100%的组合包括AFP+AFU+GGT、AFP+AFU+ALP、AFP+GGT +ALP、AFU+GGT+ALP、AFP+AFU+GGT+ALP、AFP+AFU+GGT+CA199、AFP+AFU+GGT+ALP+ CA199、AFP+AFU+GGT+ALP+CA199+CA50;总阳性似然比高的组合有AFP+AFU、AFP+AFU+GGT、AFP +AFU+ALP,分别为57.9、52.9、53.6。结论PHC中合理的TM检测组合为AFP+AFU+GGT或AFP+AFU +ALP。更多还原

【Abstract】 Objective To screen reasonable panel of serum tumor markers(TM)for primary hepatic carcinoma (PHC).Methods Eight kinds of TM includingα-fetoprotein(AFP),α-L-fucosidase(AFU),carcinoembryonic anti- gen(CEA),carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125),carbohydrate antigen 50(CA50),γ-glutamyhransferase(GGT)and alkaline phosphatase(ALP)in serum were tested in 50 patients with PHC,40 patients with cirrhosis and 60 healthy controls by electrochemical luminescence assay,immunoradiometric assay and continuous monitoring assay.Parallel test and series test were used and analyzed.Results 100% sensitivity in parallel test were found in panel of AFP+AFU+GGT,AFP+AFU+ALP,AFP+GGT+ALP,AFU+GGT+ALP,AFP+AFU+GGT+ ALP,AFP+AFU+GGT+CA199,AFP+AFU+GGT+ALP+CA199 and AFP+AFU+GGT+ALP+CA199+CA50. The positive likelihood ratio of AFP+AFU,AFP+AFU+GGT and AFP+AFU+ALP were 57.9,52.9 and 53.6 re- spectively,which were higher among all the panel.Conclusions Considerably,the reasonable panel of serum TM for PHC was AFP+AFU+GGT or AFP+AFU+ALP.更多还原
