【摘要】 目的探讨底节区高血压脑出血外科手术方法和效果。方法回顾性分析采用改良翼点颞中回入路治疗35例底节区高血压脑出血病例。结果术后24h内复查头颅CT明确血肿清除效果,30例血肿清除>90%,4例血肿清除<70%,1例再出血。术后存活34例,死亡1例,为再出血。存活者随访6个月,根据GOS评分:恢复良好18例,中残11例,重残3例,植物生存1例,死亡1例。结论改良翼点颞中回入路治疗底节区高血压脑出血效果好,具有止血彻底;血肿清除率高、神经功能恢复好等优点。更多还原

【Abstract】 Objective To study the method of and effect on treatment of patients with hypertensive basal ganglia hematomas through modified transpterional middle temporal gyrus approach. Methods Thirty-five paitients with hypertensive basal ganglia hematomas underwent treatment through modified transpterional middle temporal gyrus approach and the clinical data were retrospectively analysed. Results The postoperative CT scan showed that in 30 cases the removel rate of the hematomas amounted to more than 90%, while in 4 cases less than 70%. Thirty-four paitients survived and 1 paitient died after operation.The therapeutic effects of the survival were evluated 6 months after operation according to Glasgow outcome scale(GOS) categories: There were good recovery in 18 cases,moderate disability in 11 cases, severe disablement in 3 cases, persistent vegetative state in 1case, and death in 1 case.Conclusion The clinical effect of the operation on the hypertensive basal ganglia hematomas through modified transpterional middle temporal gyrus approach is satisfactory, and it has such strong points as thorough hemostasia during the operation, high hematomas removal rate, and good nervous function recovery.更多还原
