肿瘤坏死因子α基因多态性与肺结核性慢性阻塞性肺病的关系研究
The relationship between the gene polymorphisms of TNFαand the genetic susceptibility to chronic obstructive pulmonary disease (COPD) with tuberculosis
【摘要】 目的探讨肿瘤坏死因子α(TNFα)基因-308G→A多态性与中国汉族人肺结核性慢性阻塞性肺病(COPD)的相关性。方法采用病例对照研究方法,应用聚合酶链反应-限制性片断长度多态性(PCR-RFLP技术,检测患有肺结核及合并COPD患者106例和健康对照者108例TNFα基因启动子-308G→A多态性,对其易感性进行分析,同时检测血清TNFα含量。结果肺结核组及合并COPD组TNFα基因GA杂合子共29人,基因型频率分别为22.7%、30.6%,分别与对照组(11.1%)相比明显升高。肺结核组及COPD组TNFαA等位基因频率分别为0.114、0.153,显著高于对照组(0.056),差异有显著性(χ2=4.82,P<0.05及χ2=6.78,P<0.01)。携带TNFαA等位基因的COPD个体比不携带此等位基因的COPD个体发生COPD的危险性增加近5倍,RR=5.96(95%CI 0.95,~7.85)。在肺结核性合并COPD患者,治疗前TNF浕含量明显高于治疗后,携带TNFαA等位基因者比非携带TNFαA等位基因者TNF浕含量明显升高,有差异极显著性(P<0.01)。结论TNFα基因-308G→A多态性与COPD的易感性相关,携带TNFαA等位基因者,可增加肺结核感染者发生COPD的危险性。
【Abstract】 Objective To study the relationship between the gene polymorphisms of TNFαand the genetic susceptibility to chronic obstructive pulmonary disease(COPD)with tuberculosis. Methods The gene polymorphism of TNFα-308G→A in 106 cases of COPD with tuberculosis and 108 controls were detected by means of PCR-RFLP techniques combined with case control study.Results The frequency of TNFα GA genotype increased in tuberculosis group and COPD group compared with controls(22.7%,30.6% vs.11.1%,respectively),the frequency of TNFα A allele increased in tuberculosis group and COPD group in comparison with controls(0.114,0.153vs0.056,χ2=4.82,P<0.05,χ2=6.78,P<0.01).RR=5.96(95%CI 0.95,~7.85).quality of TNFɑin COPD with tuberculosis was increased in patients with TNFα A allele compared without TNFα A allele(P<0.01).It was suggested that the gene polymorphism of TNFα be associated with genetic susceptibility to COPD with tuberculosis.Conclusions The genetic susceptibility to COPD with tuberculosis was associated with TNFα-308 A allele,TNFα A allele carrier in the patients with tuberculosis have an increased risk of the occurrence for the chronic obstructive pulmonary disease.
