联合检测端粒酶和癌胚抗原对良恶性胸腔积液的诊断价值的研究

【摘要】 目的:探讨联合检测肿瘤标志端粒酶和癌胚抗原(CEA)对良恶性胸腔积液的诊断价值。方法:选择恶性胸腔积液31例,非恶性胸腔积液32例,采用聚合酶链反应-酶链免疫吸附分析法(PCR-EL-ISA)检测胸腔积液端粒酶活性,用酶免疫分析法(EIA)检测胸腔积液CEA水平,并对测定结果进行统计学处理。结果:端粒酶活性测定诊断恶性胸腔积液的敏感性为87.1%(27/31),特异性90.6%(29/32)。CEA诊断恶性胸腔积液的敏感性为77.4%(24/31),特异性87.5%(28/32)。结论:端粒酶活性的测定,在良恶性胸腔积液的鉴别诊断中具有重要价值,但存在假阴性和假阳性,若与胸腔积液CEA联合检测,对良恶性胸腔积液的鉴别诊断意义更大。

【Abstract】 OBJECTIVE:To investigate the diagnostics value of combining telome rase activity with CEA levels in differentiating malignant from nonmalignant pleural effusion.METHODS:Thirty-one cases with malignant and 32 cases with nonmalignant pleural effusion were enrolled into this study.The telomerase activity in pleural effusion was tested by means of PCR-ELISA,and CEA levels in pleural effusion were tested by the EIA method.All the results were analyzed by the statistical method.RESULTS:The sensitivity of telomerase activity testing for diagnosing malignant pleural effusion was 87.1%(27/31),and the specificity 90.6%(29/32).The sensitivity of CEA testing for diagnosing malignant pleural effusion was 77.4%(24/31),and the specificity 87.5%(28/32).CONCLUSIONS:The activity testing of telomerase has important value in differential diagnosis of benign and malignant pleural effusion,but false negative and false positive remain existing.The combined testing of CEA with telomerase can be more valuable in the differential diagnosis of malignant from nonmalignant pleural effusion.
