食管鳞癌组织VEGF和Angiopoietin-2表达及其临床意义的研究

【摘要】 目的:探讨血管内皮生长因子(VEGF)和血管生成素-2(Ang-2)在食管上皮肿瘤的表达及其与肿瘤分期、分级的关系。方法:应用免疫组织化学SP法检测60例食管鳞癌组织、20例食管癌旁组织及10例正常食管黏膜上皮组织中VEGF及Ang-2的表达情况,并结合临床病理资料进行分析。结果:VEGF蛋白在食管鳞癌、癌旁组织和正常食管鳞状上皮阳性表达率分别为45.0%、15.0%和10.0%,Ang-2蛋白阳性表达率分别为36.7%、10.0%和0,差异均有统计学意义,P<0.05。VEGF及Ang-2蛋白阳性表达与肿瘤浸润深度有关,T3、T4期显著高于T1、T2期,VEGF蛋白阳性表达率分别为52.1%和16.7%,P=0.049;Ang-2蛋白表达率则分别为43.8%和8.3%,P=0.041。VEGF及Ang-2蛋白阳性表达与淋巴结转移有关,有淋巴结转移者显著高于无淋巴结转移者,VEGF蛋白表达率分别为69.0%和22.6%(P=0.001),Ang-2蛋白表达率分别为62.1%和12.9%,P=0.000。结论:VEGF及Ang-2在食管上皮鳞癌组织中表达明显高于非癌组织,表明其在食管上皮鳞癌的发生、发展中占有重要作用;VEGF及Ang-2可能为食管鳞癌的标志分子;具有潜在的临床诊断价值;VEGF及Ang-2与食管癌的TNM分期有关。

【Abstract】 OBJECTIVE:To investigate the expressions of vascular endothelial cell growth factor(VEGF)and angiopoietin-2(Ang-2)in esophageal squamous cell carcinoma(ESCC)and determine their relationship with the clinical stage and histological grade.METHODS:The expressions of VEGF and Ang-2 were determined by the immunohistochemical method in sixty cases of ESCC,twenty cases of adjacent noncancerous esophageal mucosa and ten cases of normal esophageal mucosa,and their correlation with clinicopathological features was analyzed.RESULTS:The positive expression rates of VEGF in ESCC,adjacent noncancerous esophageal mucosa and normal esophageal mucosa were 45.0%,15.0% and 10.0%(P<0.05),respectively.In addition,the positive expression rates of Ang-2 were 36.7%,10.0% and 0(P<0.05),respectively.The positive expression rates of VEGF and Ang-2 were closely correlated with the tumor’s depth of invasion and lymph node metastasis.The positive expressions of VEGF and Ang-2 in the stages of T3 and T4 were higher than those in the stages of T1 or T2.The positive expression rates of VEGF in the stages of T3-4 and T1-2 were 52.1% and 16.7%(P=0.049),respectively.The positive expression rates of Ang-2 in the stages of T3-4 and T1-2 were 43.8% and 8.3%(P=0.041),respectively.The positive expressions of VEGF and Ang-2 in ESCC with lymph node metastasis were higher than those without lymph node metastasis.The positive expression rates of VEGF in patients with and without lymph node metastasis were 69.0% and 22.6%(P=0.001),respectively,and moreover,the rates of Ang-2 were 62.1% and 12.9%(P=0.000),respectively.CONCLUSIONS:The expressions of VEGF and Ang-2 are higher in the ESCC than those in the noncancerous esophageal mucosa,which indicates that it may play an important role during the development of ESCC.Both VEGF and Ang-2 may be useful molecular markers and have potential clinical diagnostic value.Both VEGF and Ang-2 relate to the stage of TMN in ESCC.
