过敏性紫癜患儿血和尿微量蛋白测定对过敏性紫癜肾炎的诊断价值

【摘要】 目的探讨血、尿微量蛋白测定对过敏性紫癜肾炎的预测和诊断价值,为过敏性紫癜(HSP)患儿肾损害的早期诊断和治疗提供依据。方法将242例HSP患儿分为尿常规阳性组(即过敏性紫癜肾炎患儿)127例和尿常规阴性组115例,另选择健康对照组130例。采用放射免疫法测定血β2-微球蛋白(血β2-MG)、尿β2-微球蛋白(尿β2-MG)、尿清蛋白(尿-Alb)和尿免疫球蛋白G(尿-lgG),并进行比较分析。结果3组儿童的血β2-MG水平间差异无统计学意义(P>0.05);尿常规阳性组、尿常规阴性组患儿的尿β2-MG水平与对照组比较差异均有统计学意义(P<0.01),尿常规阳性组与尿常规阴性组患儿的尿β2-MG水平间差异无统计学意义(P>0.05);尿常规阳性组、尿常规阴性组患儿的尿-Alb水平与对照组比较差异均有统计学意义(P<0.01),尿常规阳性组与尿常规阴性组患儿的尿-Alb水平间差异亦有统计学意义(P<0.01);尿常规阴性组、对照组儿童的尿-lgG水平与尿常规阳性组比较差异有统计学意义(P<0.01),尿常规阴性组与对照组尿-lgG水平间差异无统计学意义(P>0.05)。结论尿微量蛋白检测能较早反映肾损害的发生与否及其程度,可作为判断HSP肾损害的一个临床指标,为早期诊断和治疗HSP提供依据。

【Abstract】 Objective To explore the diagnostic value of small proteins in blood and urine in predicting and diagnosing Henoch-Schonlein purpura nephritis(HSPN) to afford grounds for early diagnosis and treatment of HSPN children.Methods Two hundred forty-two HSPN children were divided into positive urine test(PUT) group(n=127) and negative urine test(NUT) group(n=115),and another 130 healthy subjects were chosen as control group.Radioimmunoassay method was used to analyze comparatively blood β2-microglobulin(blood β2-MG),urine β2-microglobulin(urine β2-MG),urinary albumin(U-Alb) and urinary immunoglobulin G(rurine-IgG).Results No significant difference was noted in blood β2-MG among three groups(P>0.05);There was significant difference in urine β2-MG and Alb(P<0.05),but no significant difference in IgG(P>0.05),between NUT group and control group.PUT group was significantly different from the control in urine β2-MG,Alb and IgG(P<0.05);The difference was significant in Alb and IgG(P<0.05),but not in β2-MG(P>0.05),between PUT and NUT groups.Conclusion Urine small protein,which can detect the development and degree of kidney damages,can be used as a clinical indicator of HSPN kidney damages for early diagnosis and therapy.
