【摘要】 目的:在40例先天性心脏病婴幼患儿体外循环心脏手术中应用超滤技术,以评估超滤对患儿围术期肾小球功能的影响。方法:先天性心脏病手术患儿40例,随机分成对照组(C组,n=20)和超滤组(U组,n=20),测量两组患儿围术期不同时间点尿中微量白蛋白(MA)、转铁蛋白(TRF)、免疫球蛋白(IgG)的浓度并进行对比观察。结果:①所有患儿均顺利完成手术,但对照组比超滤组实入量明显增多(P<0.01);②两组尿MA、TRF、IgG的浓度均于术后第一天明显升高(P<0.01或P<0.05),达峰值后逐渐下降。两组比较,C组比U组术后三种微量蛋白升高明显,持续时间也明显延长(P<0.01或P<0.05)。结论:超滤技术对婴幼儿体外循环心脏手术后肾小球功能具有明显的保护作用。更多还原

【Abstract】 Objective:To investigate the effects of ultrafiltration on glomerular function after extracorporeal circulation(ECC).Methods:Forty congenital heart diseases(CHD) patients weighed(10～15)kg underwent open heart surgery with ECC were randomLy divided into two groups: ultrafiltration group(group U,n=20) and control group(group C,n=20).In group U,ultrafiltration was performed during rewarming period and after ECC when the homodynamic was stable.Urine was collected before operation、before rewarming and after 1st,3rd,5th operation,and level of urinary microalbumin(MA)、transferrin(TRF) and immunoglobulin(IgG) were determined.Results:①The real priming volume in group C was significantly higher than that was in group U(P<0.01).②The level of urinary MA、TRF and IgG increased significantly on the 1st after operation(P<0.01) as compared with the baseline values in two groups,then decreased gradually.But level of the MA、TRF and IgG were significantly higher in group C than these in group U after operation and kept longer time(P<0.01/P<0.05).Conclusions:Ultrafiltration has protective effects on glomerular function in infant’s patients underwent ECC.更多还原
