采用男性尿道分泌物、精液及尿液检测人乳头瘤病毒感染的比较
Comparison of urethral specimen with seminal and urinary specimens for diagnosis of human papillomavirus infection in males
【摘要】 目的选择一种易于采集、提高男性HPV阳性检出率、适合基因膜芯片法的标本类型。方法参考改进的PGMY09/11和β-球蛋白引物PC03/GH20设计PCR扩增引物,根据Genbank中的HPV型特异性序列设计及合成探针,制备可同时对18种高危型(HPV-16,18,31,33,35,39,45,51,52,53,56,58,59,66,68,73,83,MM4)和5种低危型(6,11,42,43,44)进行基因分型检测的特定PCR-膜芯片,通过杂交,进行男性HPV基因亚型检测,以此来比较男性尿道分泌物、精液及尿液3种类型标本的检出率。并对检测结果进行统计学分析。结果初筛2004年5月—2007年5月191位男性患者中42例HPV阳性标本平行检测尿道分泌物、精液及尿液3种类型标本后发现,尿道分泌物的阳性率最高,32例阳性(32/42,76.2%)。尿液标本最不敏感,仅有16例阳性(16/42,38.1%)。精液居中,27例阳性(27/42,64.3%)。尿道分泌物和尿液检测具有明显差异(P<0.05)。结论临床常规检测男性HPV感染采用尿道分泌物作为采集部位和标本是具有较高的敏感性的,并且在3种标本中采样相对简单些,是切实可行的。
【Abstract】 Objective To develop a right sorting of specimen for diagnosis of human papillomavirus infection in males. Methods Referring to improved primers PGMY09/11andβ-globulin PC03/ GH20, primers of PCR were designed. Meanwhile, according to Genbank data about HPV, probes were synthesized for detecting simultaneously 23 subtypes including 18 high-risk subtypes:HPV-16, 18, 31, 33,35, 39, 45, 51,52, 53,56,58,59,66,68,73, 83, MM4 and 5 low-risk subtypes:6,11,42,43 and 44, and they were fixed on a membrane to make DNA chip. Through hybridizing, HPV subtypes were detected in males with comparison of urethral, seminal and urinary specimens. The results were analyzed with statistical software. Results In 42 HPV positive specimens of 191 male patients, the urethral specimens were of the highest detection rate (32/42, 76.2%),the next seminal samples (27/42,64.3%), and the urinary specimens the lowest (16/42, 38.1%). Between urethral specimens and urinary, the difference was obvious with statistical significance (P<0.05). Conclusion The relatively right specimen adapted to clinical detecting HPV subtypes is urethral sample. Moreover, the sorting of sample is easily performed.
