载脂蛋白E基因多态性与颈动脉粥样硬化及相关炎症因子产生的关系

【摘要】 目的:探讨载脂蛋白E(ApoE)的基因多态性与颈动脉粥样硬化(CAS)及相关炎症因子产生的关系。方法:入选的83例对象均行彩色多普勒检测颈动脉内中膜厚度(IMT)及观察颈动脉有无斑块,根据结果分成CAS组及对照组,采用聚合酶链式反应-限制性片段多态性方法检测ApoE基因多态性,并测定总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及相关炎症因子。结果:CAS组ε3/4基因型频率明显高于对照组(P<0.05),两组间ε4等位基因频率差异有显著性(P<0.05)。不同的ApoE等位基因组间TC、LDL-C、TG、颈动脉IMT、超敏C反应蛋白(Hs-CRP)和纤维蛋白原(FIB)存在显著性差异(P<0.05),并且上述指标的值以携带ε4等位基因组为最高。结论:ApoE基因多态性是影响血清TC及LDL-C水平的重要遗传因素之一,同时与体内炎症状态相关,并且在一定程度上影响CAS病变的进程。

【Abstract】 Objective:To explore the relationship among apolipoprotein E(ApoE) genetic polymorphism,the carotid atherosclerosis(CAS) and the produce of related inflammatory factors. Methods:Total 83 patients were examined by B-mode ultrasound to measure intima-media thickness(IMT) of the common carotid artery and plaque formation. According to the results of B-mode ultrasound examination,the patients were divided into the carotid atherosclerosis group and control group. The polymerase chain reaction restriction fragment length polymorphism technology(PCR-RFLP) was used to detect ApoE genotype; Serum triglyceride(TG),total cholesterol (TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C) and inflammatory factors were measured. Results:The frequency of ε3/4 in CAS group was higher than that in control group(P < O.05). The values of TC,LDL-C,TG,carotid IMT,Hs-CRP and fibrinogen among different groups of ApoE allele were significantly different(P < O.05),and the value of these parameters in the group carrying ε4 allele ranked the highest among all genetic polymorphism. Conclusion:ApoE genetic polymorphism was one of the important genetic factors affecting serum total cholesterol and LDL-C level. Meanwhile correlating with the state of inflammation in vivo,ApoE genetic polymorphism also affected CAS process in a certain extent.
