喘息性支气管炎患儿雾化吸入同步持续气道正压通气的效果观察

【摘要】 目的探讨更适用于持续气道正压通气(NCPAP)治疗的雾化吸药方法。方法选取90例持续气道正压通气治疗的喘息性支气管炎患儿,观察组45例采用无创鼻塞持续气道正压通气与雾化吸药同步进行,对照组45例采用常规的雾化吸药(雾化时停止持续气道正压通气)。比较两组治疗前、后患儿血气分析变化、总通气时间和鼻黏膜损伤、雾化过程中发生呼吸困难的例数。结果观察组治疗后血气分析中PaO2、PaCO2、SaO2的改善较对照组更明显(P<0.01),且观察组的平均持续气道正压通气时间、呼吸困难例数比对照组少(P<0.01);两组发生鼻黏膜损伤例数的差异无统计学意义(P>0.05)。结论雾化同步持续气道正压通气的方法能明显提高PaO2和降低PaCO2,更适用于持续气道正压通气治疗的喘息性支气管炎患儿。

【Abstract】 Objective To explore the proper method of aerosol inhalation during nasal continuous positive airway pressure ventilation(NCPAP).Methods 90 asthmatic bronchitis infants were equally assigned into the observation group and the control group.In the former group,aerosol inhalation side by side with NCPAP was used and in the control,only aerosol inhalation was used.Blood gas changes,total duration of ventilation,damage of nasal mucosa and cases with anhelation during aerosol inhalation were compared between the groups.Results There were more improvements in PaO2,PaCO2,SaO2 in the observation group than in the control group(P<0.01).The observation group had significantly shorter time for ventilation and fewer cases with anhelation(P<0.01).But the difference was insignificant between the groups in terms of cases with damage of nasal mucosa.Conclusion The method of aerosol inhalation in the observation is more likely to increase PaO2 and decrease PaCO2,so it is more fitful for the treatment with NCPAP.
