【摘要】 目的探讨系统性红斑狼疮(SLE)患者血清中抗中性粒细胞胞浆抗体(ANCA)的阳性检出率及其与病情的相关性。方法应用间接免疫荧光技术(IIF)检测。结果108例SLE患者中,ANCA阳性35例(32.5%),皆为核周型ANCA(P-ANCA),而且滴度越高临床病情越重。35例ANCA阳性组中,狼疮活动为28例(80%),73例ANCA阴性组中活动35例(47.9%),两组阳性率相比,差异有显著性(P<0.05)。ANCA阳性组伴肺间质病变、肾脏病变明显高于阴性组,均有统计学意义(P<0.05)。正常对照20例均为阴性。结论ANCA在SLE患者中有一定的阳性检出率,且与疾病的活动及肾脏受损、肺间质病变具有相关性。更多还原

【Abstract】 Objective To investigate the clinical significance of antineutrophilic cytoplasmic antibodies in patients with systemic lupus erythematosus.Methods The expression of ANCA,anti-double-stranded DNA antibodies,and complements in 108 lupus patients were tested by indirect immunofluorescence(IIF).One hundred and eight lupus patients had chest radiography and lung high-resolution computed tomography(HRCT) and 24 hours urine protein done to help evaluating the severity of disease.Results The expression of ANCA in serum of 35 lupus patients was posive(all were perinuclear ANCA) with a postive rate of 32.5%.The 35 ANCA-postive lupus patients had higher titers of anti-dsDNA antibodies,lower levels of complements(CH50,C3,C4) and higher erythrocyte sedimentation rate compaed to ANCA-negative patients(P<0.05).In 35 ANCA-postive patients,28 had interstitial lung disease(80%).In the other 73 ANCA-negative patients,35 had interstitial lung disease(47.9%).The prevalence of interstitial lung disease of ANCA-postive patients was higher than ANCA-negative patients(P<0.05).In 35 ANCA-postive patients,25 had kidney disease(71%).In the other 73 ANCA-negative patients,30 had kidney disease(41%).The prevalence of kidney disease of ANCA-postive patients was higher than ANCA-negative patients(P<0.05).Conclusion The positive expression of ANCA in lupus patients is related to more active disease,and higher frequency of kidney disease and interstitial lung disease.更多还原
