血脂代谢紊乱与外周血白细胞C5a受体表达的关系

【摘要】 探讨血脂代谢紊乱与外周血白细胞C5a受体表达的关系。方法高脂血症患者60人,同期正常对照30人,测定血清总胆固醇、甘油三酯、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、脂蛋白(a)、氧化低密度脂蛋白(oxLDL)等脂代谢指标以及补体C3、C4,使用流式细胞仪检测外周血白细胞C5a受体,分析脂代谢异常以及补体C3、C4水平与C5a受体表达的关系。结果高脂血症组总胆固醇、甘油三脂、LDL-C、oxLDL、脂蛋白(a)、补体C3、C4等较对照组明显升高;高脂血症组CD88阳性淋巴细胞、单核细胞、粒细胞平均荧光强度有升高趋势(3.05±0.91比2.964±0.76、4.90±1.33比4 58±1.13、13.87±4.77比13.604±3.54).但差异无统计学意义(P>0.05);直线相关分析,CD88阳性单核细胞及粒细胞平均荧光强度与脂蛋白(a)正相关,CD88阳性单核细胞及粒细胞平均荧光强度与oxLDL亦呈正相关,与总胆固醇、甘油三脂、LDL-C、HDL-C、补体C3和C4不相关;偏相关分析及多元回归显示oxLDL是CD88阳性单核细胞及粒细胞平均荧光强度的独立预测因素。结论oxLDL可能影响外周血白细胞C5a受体的表达。

【Abstract】 Objective To investigate the relationship between lipid disorders and the expression of CSa receptors on circulating white blood cells.Methods We selected 60 consecutive patients with hyperlipidemia and 30 healthy people during the same period as controls.The expression of C5a receptors on white blood cells were tested by flow cytometry in all subjects,and it’s association with total cholesterol,triglyceride,low density lipoprotein eholesterol(LDL-C),high density lipoprotein eholesterol(HDL-C),lipoprotein(a),oxidized low density lipoprotein(oxLDL) and complement component C3,C4 was analyzed.Results Compared with the controls,the mean blood total cholesterol,triglyeerides,LDL-C,oxLDL,lipoprotein(a),and complement C3,C4 were markedly increased in patients with hyperlipidemia.Mean fluorescent intensities(MFIs) of C5a expressing lymphocytes,monocytes,and granulocytes showed a tendency of elevation(3.05±0.91 vs 2.96±0.76,4.90±1.33 vs 4.58±1.13,13.87±4.77 vs 13.60±3.54),but the differences were statis- tically insignifieant(P>0.05 for all).MFIs of C5a expressing monocytes and granulocytes were positively correlated with both lipoprotein(a) and oxLDL,but not with total cholesterol,triglyc- eride,LDL-C,and complement C3,C4.Furthermore,oxLDL was the independent predictor for MFIs of C5a expressing monocytes and granulocytes.Conclusion oxLDL might affect the ex- pression of C5a receptors on circulating monocytes and granulocytes.
