广角激光检眼镜在准分子激光矫正手术前检查中的应用价值
To evaluate the diagnostic value of the panoramic 200 scanning laser ophthalmoscope in screening for retinopathy of myopic patients before excimer laser corneal surgery
【摘要】 目的评价广角激光检眼镜(全景200)在准分子激光术术前检查中的应用价值。方法对拟行准分子激光手术治疗的近视患者349例(693只眼),利用全景200在自然瞳孔正常照明条件下获取眼底图像结果进行分析,并与散瞳双目间接检眼镜检查结果进行比较。结果349例(693只眼)全景200发现了有较大临床意义的眼底病变28例(35只眼),近视患者有意义的眼底改变包括发生在周边视网膜的格子样变性、霜样变性、裂孔及局限性视网膜脱离。其中格子样变性4例(5只眼),霜样变性12例(15只眼),视网膜裂孔6例(7只眼),局限性视网膜浅脱离7例(7只眼),视网膜脱离伴视网膜下增生1例(1只眼)。这些病变随着近视屈光度的增加发病率明显增高(P<0.005)。经瞳眼底检查发现38例(47只眼)存在眼底病变,全景200对眼底疾病筛查的敏感度达到74.47%(35/47)。有18例(24只眼)于激光光凝治疗后复查了全景200检查,均能清晰显示光凝斑。结论全景200在准分子激光矫正手术前眼底病变的筛选中,具有免散瞳、快速、大范围眼底观察等优点,可保存客观记录,提供高分辨率的图像,对近视患者的周边眼底病变有较高的检出率,并能提供连续性眼底变化监测。
【Abstract】 Objective To evaluate the value of the panoramic200 scanning laser ophthalmoscope(Panoramic200) in preoperative fundus examination of laser in excimer laser surgery.Methods Images of ocular fundus of 693 eyes of 349 myopic patients before excimer laser surgery were obtained by panoramic200 scanning laser ophthalmoscope(Panoramic200),which were compared with the results of binocular indirect ophthalmoscopy(BIO).Any discrepancies of the results of the two methods were then analyzed.Results 35 eyes of 28 patients with retinopathy were analyzed using the optomap images with the panoramic 200,including lattice degeneration in peripheral retina(5 eyes in 4 patients),frosty degeneration(15 eyes in 12 patients),tears retina(7 eyes in 6 patients),regional detachment of retina(7 eyes in 7 patients) and retinal detachment with intercurrent sub-retinal proliferation(1 eye).There seems to be a ascent trend for pathologic retina with the increase of refractive error.47 eyes with pathologic retina in 38 patients were diagnosed by BIO.The sensitivity of panoramic200 for retina disease screening reached 74.47%(35/47).24 eyes in 18 patients accepted follow-up panoramic 200 examination,and the laser light spots were clearly obtained.Conclusion The optos panoramic200 scanning laser ophthalmoscope which utilizes non-mydriatric,rapid,ultra widefield technologies,can take high-resolutions retinal images and save external records.It is also a valuable method in screening for peripheral retinopathy of myopic patients before excimer laser surgery and in monitoring the changes in fundus continously,
