乳腺癌外周血角蛋白-19的检测及其临床意义
Molecular Detection and Prognostic Value of Cytokeratin-19 in the Peripheral Blood in Patients with Breast Cancer
【摘要】 目的:研究反转录酶链聚合反应(RT-PCR)检测乳腺癌患者外周血角蛋白-19(Keratin-19,CK-19)的临床意义。方法:2002年9月—2003年10月,共入选53例乳腺癌患者。分别在术前、术后、第1次化疗结束后以及术后9~12个月,应用反转录酶链聚合反应(RT-PCR)检测血中的CK-19。结果:53例乳腺癌患者中,Ⅰ期、Ⅱ期和Ⅲ期患者CK-19阳性率分别为8.7%,31.82%,71.43%。手术后复查,Ⅱ期患者增加1例。经第1次化疗后,各期原CK-19阳性患者分别减少为2例、3例和2例。结论:运用反转录酶链聚合反应(RT-PCR)检测外周血CK-19敏感性高、特异性好,能成为乳腺癌的一个很好的预后指标,同时亦可用于对化疗的评估。
【Abstract】 Objective:To explore the feasibility of using the reverse transcriptase-polymerase chain reaction (RT-PCR) technique to detect the cytokeratin 19 (CK19) in peripheral blood of breast cancer patients, and to evaluate the effectiveness of the chemotherapy by detect the micrometastasis in peripheral blood.Methods:The levels of CK-19 were evaluated by using RT-PCR at the time of preoperative, postoperative, after the first treatment with chemotherapy and 9-12 months after operation in 53 cases with breast cancer from September 2002 to October 2003. Results: According to World Health Organization tumor staging methods, there was 1 patients in staging 0, 23 patients in staging one, 22 patients in staging two and 7 patients in staging three. Every staging the number of Ck-19(+) patients was 0, 2, 7, 5. And postoperative only increased 1 person (at staging two). After the first treatment with chemotherapy, the number of Ck-19(+) patients decreased to 0, 2, 3, 2. and 9-12 months after operation, the number of Ck-19(+) patients decreased to 0, 2, 1, 2.Conclusion:It is a high sensibility and specificity method using RT-PCR to detect CK-19 in peripheral blood of breast cancer patients; It will be a good marker which can find the subclinical metastasis in breast cancer; And It may help the chemotherapy for the patients.
