足细胞损伤对IgA肾病肾小球硬化的作用
Glomerular Podocyte lesion in IgA Nephropathy
【摘要】 目的:研讨IgA肾病患者肾小球足细胞损伤对IgA肾病进展的作用。方法:44例经肾穿刺活检明确诊断的IgA肾病患者,采用免疫组织化学技术定量肾组织病理并借助足细胞表面特异性标记物Wilms′肿瘤蛋白(WT1)对肾小球足细胞进行准确的密度定量分析。结果:WT1表达在正常肾脏肾小球的脏层上皮细胞,IgA肾病患者随着病变加重,硬化肾小球增加,足细胞数目也明显减少(P<0.01),肾小球中的WT1表达与尿蛋白定量无明显相关性,但与血清肌酐水平呈显著正相关(r=0.541,P<0.01)。结论:肾小球足细胞损伤可能是影响IgA肾病进展的因素之一。
【Abstract】 Objective:To explore the lesion of glomerular podocyte in the renal tissue of patients with IgA nephropathy.Methods:Forty-four type IgA nephropathy with biopsy-proven IgA nephropathy were used in this study.Immunohistochemistry were applied to detect the expression of Wilm’s tumor(WT1),a marker of podocyte,in the renal tissue of patients with IgA nephropathy.The density of podocyte per glomerulus was calculated using specific software.Results:WT1was normally expressed in the glomerular endothelial cells.The expression of WT1 was significant decreased in sclerotic glomeruli of patients with IgA nephropathy(P<0.01).WT1 expression did not correlate with proteinuria,but distinctly associated with the level of serum creatinine(P<0.01).Conclusion:The lesion of glomerular podocyte may contribute to the progression of IgA nephropathy.
