带胫后动脉内踝上皮支转移皮瓣13例报告
The Application Report of 13 Case Skin Flap from Upper Cutaneous Branches of Inner Malleolus of Posterior Tibial Artery
【摘要】 目的:探讨带胫后动脉内踝上皮支转移皮瓣修复足踝部皮肤软组织缺损的临床效果。方法:从2002年1月-2007年3月,采用以胫后动脉内踝上皮支为蒂的逆行岛状小腿内侧转移皮瓣修复足跟后部软组织缺损3例、内踝部软组织缺损4例、踝前与足背软组织缺损6例。结果:临床应用13例,皮瓣面积最小为8cm×4cm,最大18cm×7cm,术后13例皮瓣全部成活,随访11例,随访时间2~18个月,皮瓣保护性感觉恢复,质地良好,外形不臃肿。结论:内踝上皮支皮瓣厚薄适中,手术切取容易,质地好,血运可靠,不牺牲胫后血管,成活率高,是修复足踝部软组织缺损的理想皮瓣。
【Abstract】 Objective:The clinical outcome of skin flap from upper cutaneous branches of inner malleolus of posterior tibial artery to repair skin and soft tissue defect from ankle was studied.Methods:3 case skin and soft tissue defect of postero-heelstick, 4 case skin and soft tissue defect of inner malleolus and 6 case skin and soft tissue defect of fromantero-ankle and dorsum pedis were repaired by retrogression island-form skin flap used upper cutaneous branches of inner malleolus of posterior tibial artery as a pedicel from January 2002 to March 2007. Results:13 case were operated. The size of skin flap ranged form 8 cm×4 cm to 18 cm×7 cm. All the skin flaps were alive after operation. 2-18 months follow-up for 11 case , preserved sensory of skin flap was recovery. The skin flap had a good texture without fat and clumsy.Conclusion:The skin flap from upper cutaneous branches of inner malleolus of posterior tibial artery has suitable thickness , good texture and easier to be cut. The blood transport of skin flap is safety without blood vessel loss. The flap which has a high survival rate is ideal to repair skin and soft tissue defect around ankle.
