【摘要】 目的探讨合并糖尿病的腹部外科患者的围手术期处理措施。方法对48例腹部外科手术患者并存糖尿病时围手术期诊断与治疗进行回顾性分析。结果糖尿病以2型多见,老年患者占68.8%,病情隐匿,术前漏诊2例。48例患者中非急诊手术37例,急诊手术11例。术后发生酮症酸中毒3例,切口感染5例,肺部感染6例,泌尿系感染2例,真菌感染1例,腹部切口裂开2例,多脏器功能衰竭死亡1例。结论对合并糖尿病的腹部外科患者,围手术期严格控制血糖,合理使用抗生素,选择合理手术时间和麻醉方式,有效的营养支持,是外科手术治疗成功的有效措施。更多还原

【Abstract】 Objective To investigate the perioperative treatment to the patients with diabetes mellitus in abdominal surgery.Methods Forty-eight patients with diabetic patients undergoing various surgical procedures were retrospectively analyzed.Results The associated diabetes occurred frequently in type 2,elderly patients(68.8%).Two cases were missed diagnosis owing to the insidious diabetic courses.In total 48 cases,11 were subjected to emergency surgery and 37 non-emergency surgery respectively.After surgery,3 cases suffered from the ketoacidosis,5 cases from the infection of incisions,6 cases from pulmonary infection,2 cases from the infection of urinary system,1 case from fungal infection,2 cases from disruption of abdominal wound and 1 died of multiple visceral organ failure.Conclusion For the patients with diabetes mellitus subject to abdominal surgery,the blood sugar should be strictly controlled,antibiotics properly used,the opportunity for surgery and the means of anaesthesia properly chosen,and effective nutrition support given.更多还原
