混悬聚乳酸/磷酸三钙复合材料修复下颌骨缺损的实验研究
Using PDLLA/TCP,PDLLA Composite to Repair Mandibular Defect.
【摘要】 目的:观察PDLLA/TCP复合物修复大鼠下颌骨缺损的愈合过程,探讨该复合物作为下颌骨缺损修复替代材料的可行性。方法:在60只大鼠一侧下颌骨体部形成下颌骨缺损模型,并随机分为3组,每组20只。分别为PDLLA组,PDLLA/TCP组及空白对照组。其中空白对照组不植入任何物质。术后第2、4、8、12、16周分批处死各组大鼠,取下颌骨观察缺损部位愈合情况,并行X线检查、组织学切片观察。结果:创口观察显示术后第1周,各组大鼠伤口愈合良好,创周皮肤无红肿,无脓肿形成。术后第2周,空白组1例在手术部位出现炎性包块,后自行吸收,未形成瘘道。后期各组大鼠伤口均正常愈合。组织学观察显示PDLLA/TCP组和纯PDLLA组早期有大量纤维结缔组织形成和炎性细胞浸润,随后新生的纤维结缔组织将材料分隔成块状,缺损区出现大量新生骨岛,同时可见丰富的成纤维细胞和成骨细胞。PDLLA/TCP组新骨形成速度快于纯PDLLA组,空白组新骨形成数量明显少于各材料充填组。结论:PDLLA及PDLLA/TCP复合材料均有较强刺激成骨的作用,PDLLA/TCP复合材料修复骨缺损效果好于纯PDLLA,更适合用于下颌骨缺损修复。
【Abstract】 Objective: To investigate the effect of PDLLA or PDLLA/TCP composite on mandibular defect reparation.Methods: The model of mandibular defect was created in sixty Wistar rats.Twenty defects were filled with PDLLA/TCP composite;twenty were filled with pure PDLLA;the others were filled with nothing——-as the control group.The clinical observation,X ray photography,histological observation were taken at the 2nd,4th,8th,12th,16th week after mandibular defect filled with PDLLA or its composite.Results: All rats recovered well.Local inflammation was found in control group at 2nd week after operation and recovered later.The histological study showed that osteoblasts and fibroblasts were more active in test group in the early healing stage than in the control.The filled materials were divided into several parts by new fibro tissues.Large numbers of new bone islands were observed in the defect areas of test group.Conclusion: Both pure PDLLA and PDLLA/TCP composite can repair rats mandibular bone defect.Bone grows faster in PDLLA/TCP composite group than in the pure PDLLA group.
