【摘要】 目的探讨牙周引导组织再生术(GTR)与骨移植治疗牙周骨缺损的失败原因和防治对策.方法自1999年至2007年用GTR与骨移植治疗牙周骨缺损131例175牙,随访0.5～1 a,失败19例20牙,对失败病例进行分析.结果术后感染3例,牙周溢脓骨移植物溢出8例(9牙),龈瘘1例,根折4例,牙龈坏死1例,复发性根尖炎2例.结论GTR与骨移植治疗牙周骨缺损的疗效受病例选择、骨缺损形态、膜和植骨材料类型、技术因素、口腔护理、全身状态等多因素影响,采取必要的防治措施是治疗成功的关键.更多还原

【Abstract】 Objective The purpose of this study is to discuss the reasons for the failure in the treatment of periodontal bone defects and the measures of the prevention and control of periodontal guided tissue regeneration(GTR) and bone transplantation.Methods 20 teeth of 19 unsuccessful cases with a 0.5～1 follow-up study,out of 185 teeth of 131 cases of periodontal bone defection by using GTR and bone transplantation from 1999 to 2007,were analyzed in this research.Results Among 19 unsuccessful cases,three cases were caused by postoperative infection,eight cases by bone graft overflow because of periodontal abscess(nine teeth),one case of gingival fistula,four cases of root break,one case of gingival necrosis,and two cases of apicitis.Conclusion The curative effect of GTR and bone transplantation in the treatment of periodontal bone defects is affected by the following factors: the choice of cases,bone defects form,film and the types of bone graft material,technical factors,oral care,general status,etc.So,to take the necessary prevention and control measures is the key to success.更多还原
