【摘要】 目的探讨经桡动脉途径穿刺全脑血管造影的可行性、安全性及相关问题。方法选择27例无手术禁忌证的患者,在局麻下以桡动脉为穿刺点,采用Seldinger技术,在0.035英寸超滑导丝引导下,用4F SimmonsⅢ型导管,行全脑血管造影。结果27例患者中25例顺利完成手术,成功率92.6%,1例桡动脉穿刺失败者改穿同侧肱动脉成功完成手术。1例因严重的动脉硬化而血管过于纡曲,导管未能进入主动脉弓。无手部缺血、桡动脉搏动消失及其他严重并发症。结论经桡动脉进行全脑血管造影操作安全可行,成功率高、并发症少,对适宜病例可作为首选途径,值得推广应用。更多还原

【Abstract】 Objective To study the safety,feasibility of total cerebral angiography through transradial approach.Methods A total of 27 patients with aneurysmal subarachnoid hemorrhage without contraindications for cerebral angiography were selected.After local administration of 1% lidocaine,the radial artery was punctured and a 4 F sheath was placed within the radial artery using seldinger technique and followed by angiography with 4 F Simmon Ⅲ catheters.Results Technical success was achieved in 25 patients,and the successful rate was 92.59%(25/27).One case underwent ipsilateral transbrachial angiography due to failure of transradial approach,while catheter could not transit aortic arch for severe angiosclerosis in another patient.All patients presented a palpable pulse after procedure with no symptoms and signs of hand ischemia or other major complications.Conclusion Transradial cerebral angiography is safe and feasible with higher success rate,less complications and more comfort,and can be the first choice for those who are eligible for this approach.更多还原
