完全与不完全离断兔小腿静脉动脉化吻合口组织结构变化及临床意义

【摘要】 目的观察完全与不完全离断兔小腿静脉动脉化后吻合口处血管及周围组织结构的变化规律,并与传统断指(肢)再植手术方式缝接处血管进行比较,为临床选择断指(肢)再植手术方式提供实验依据。方法用15只兔分3组:A组为完全离断小腿胫后动脉缝接胫后静脉组,B组为部分离断小腿胫后动脉缝接胫后静脉组,C组为完全离断小腿应用传统的胫后动脉缝接胫后动脉对照组,3组均结扎胫前动脉,保留胫前静脉等未动脉化的静脉及缝接其它离断软组织。术后28d麻醉下切取各组动静脉血管吻合口及其近、远端各5mm长血管和周围组织制成观察标本,经HE、免疫组化、墨汁灌注等方法处理后进行光、电镜观测。结果A组动脉化的静脉壁中膜平滑肌较B组的多,而呈圆形内皮细胞核均较B组少。A组吻合口处血管周围结缔组织增生较B组少,较C组多。结论动脉化的静脉壁主要结构成分与结缔组织增生程度有相应一致的变化关系。

【Abstract】 Objective To observe the tissue structure around seam blood vessels of arterialized vein of the omplete and uncomplete transected posterior limb of rabbit. Methods Fifteen rabbits were divided into three groups: group A( posterior limbs was completely cut off and posterior tibia artery was ligated with posterior tibia vein), group B( posterior limb was uncompletely cut off and posterior tibia artery was ligated with posterior tibia vein), group C(posterior limb was completely cut off and traditional operation type was applied to ligate the posterior tibia artery with posterior tibia artery(control group). The anterior tibia artery was ligated, anterior tibia vein was kept standing in all groups . 28 days later, blood vessels and surrounding tissue near the seam were observed by HE staining and chinese ink and immunohistochemistry under light and transmission electron microscopy. Result More smooth muscles in the wall of the arterialized vein were observed in group A than in group B, but with less number of the spherical endothelial nuclei. The proliferation of the connective tissue around the sutured blood vessels was found less in group A than in group B and more in group A than in group C. Conclusion The changes of the tissue structure of the arterialized vein wall were correspondent to the proliferation of the connective tissue.
