【摘要】 目的分析不同病期的糖尿病肾病(DN)合并高血压时,肾内血流的彩色多普勒超声血流参数的变化规律。方法将病人分组后,应用彩色多普勒超声仪进行肾脏的彩色多普勒能量图(CDEI)的扫描;并观测肾血流收缩期峰值流速(VS),舒张末期峰值流速(VD),平均流速(VM),搏动指数(PI)及阻力指数(R I)的变化。结果DN各期组的R I及PI均增高,仅DN早期组和临床DN期组与正常对照组比较增高显著(P<0.05);DN各期组间的R I及PI比较,差异均显著(分别为P<0.05、P<0.01);DN各期合并高血压、高血脂的血管R I与血压增高、血脂正常组比较,差异显著(P<0.01),DN各期合并高血压、糖尿病视网膜病变(DR)者与血压增高、无DR者比较有显著性差异(P<0.01)。结论DN各期合并高血压时患者肾血管R I的改变是判定肾内血流状况的重要依据。更多还原

【Abstract】 Objective To analyse the regularity of changes in parameters of the blood flow of renal vessels at different stages in diabetic nephropathy(DN) complicating hypertension.Methods Patients were divided into different groups,then all patients underwent renal color Doppler energy imaging(CDEI) examinations.The changes of systolic peak velocity(VS),diastolic peak velocity(VD),mean glow velocity(VM),pulsatility index(PI) and resistent index(RI) of renal blood flow were observed.Results The RI and PI of all DN groups increased,only the values of pristine DN group increased more significantly than the normal group(P<0.05);the RI and PI of differed significantly from each other among the DN groups(P<0.05,P<0.01,respectively);The RI of DN groups complicating hypertension and hyperlipemia differed significantly from the groups with normal blood pressure and blood pressure(P<0.01),the RI of DN groups complicating hypertension and diabetic retinopathy(DR) differed significantly from the groups with normal blood pressure and no DR(P<0.01).Conclusion The changes of the RI of renal vessels at different stages in diabetic nephropathy complicating hypertension are important evidences to assess the state of the renal blood flow.更多还原
