门控心肌灌注显像诊断冠心病的临床价值
The Value of 99mTc-MIBI Gated SPECT in Detection of Coronary Artery Disease
【摘要】 目的:探讨腺苷负荷99mTc-MIBI门控心肌灌注显像(G-MPI)后室壁运动和增厚率异常对冠心病的诊断价值。方法:对91例确诊或临床疑诊为冠心病患者行静息-腺苷负荷99mTc-MIBI G-MPI和非门控心肌灌注显像(MPI),两周内行冠状动脉造影(CAG),对其结果进行对比分析。结果:MPI诊断冠心病的灵敏度为85.3%,特异性为73.9%,G-MPI诊断冠心病灵敏度为92.6%,特异性为87.0%,结合腺苷负荷后局部室壁运动和增厚率评分可提高G-MPI诊断冠心病的灵敏度和特异性。结论:静息-腺苷负荷99mTc-MIBI G-MPI对冠心病的诊断有较高的灵敏度和特异性。
【Abstract】 Objective:To study the value of regional wall motion and thickening abnormalities on adenosine 99mTc-MIBI gated SPECT(G-MPI) in detection of coronary artery disease.Methods:Ninety-one patients underwent the rest-adenosine stress 99mTc-MIBI gated SPECT(G-MPI) and non-gated SPECT(MPI),all of the cases accepted coronary angiography in two weeks,and the results were compared.Results: The sensitivity of MPI and G-MPI were 85.3% and 92.6% respectively,and the specificity of MPI and G-MPI were 73.9% and 87.0% respectively.The abnormalities of regional wall and thickening had a high positive predictive value in detection of coronary artery disease,which enhanced the sensitivity and specificity of G-MPI in detection of coronary disease.Conclusion: Rest-adenosine stress 99mTc-MIBI G-MPI has high specificity in detection of coronary artery disease.Combination with regional wall motion and thickening enhances its sensitivity and specificity in detection of coronary artery disease.
